13 Beverage lines observed in drinking ice behind the main bar. Facility was instructed to use the ice bin without the beverage lines.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Unit to be serviced and facility is using the 3-bay sink until the unit is serviced.
41 Food dispensing utensil in the ice observed in direct contact with food and not having handle stored above the top of the food. Common bowls or food containers observed without handles in direct contact with bulk food ingredients (basement).
45 Crates being used for elevation in the walk-in cooler were not smooth and easily cleanable.
46 Food facility lacks sanitizer test strips to monitor chlorine sanitizer concentration.
47 General cleaning is needed in the interior surface of the basement freezer.
48 Hot water generation fluctuates due to dishwasher. Note: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food facility.
54 Light bulb was missing under the exhaust hood.

20 Potentially hazardous foods in the cold-prep unit by the cook line and pizza oven (arugula, zucchini, sauces, sausage, etc.) were held at 50-63 degrees F rather than 41 degrees F or below as required.
26 Working containers in pizza kitchen area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. Bottle was labeled.
36 Small flies were observed in the drink/wait station and large flies were observed in the P2 lounge.
42 Dishes observed being cloth dried. Note: After cleaning and sanitizing, equipment and utensils may not be cloth-dried, except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
50 Condensate from the air conditioning unit was draining into the handwash sink in the pizza prep area which is not approved.
51 Dust accumulation observed on the vent covers in the restrooms.
54 Lights were not shielded from breakage in the bread prep area/unit.
