14 Pink residue and mold observed on interior surfaces of soda gun holster. Corrective action was taken and holsters were washed, rinsed and sanitized.

8 -Paper towel dispenser empty at the handwash sink in the warewash area. Paper towels were provided. -No sign or poster posted at the handwash sink in the 3rd floor mens restroom to remind food employees to wash their hands. A handwash sign was provided. -The handwash sink in the back bar area was blocked by cases of alchol and not accesible at all times for employee use. Person in Charge relocated items.
13 Bar ice observed uncovered. Person in Charge took corrective action and covered ice.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Sanitarian advised Person in Charge to discontinue using dishwasher and use 3-basin warewash sink until dishwasher is fixed.
14 -Black residue, pink slime observed on the interior of the ice machine bins around baffle plate. All ice was discarded and Sanitarian advised person in Charge to wash, rinse and sanitize surfaces. -Pink slime observed inside gun holsters. Person in Charge removed holster for cleaning. -Residue observed on interior surfaces of food preparation sink. Dirty equipment observed in preparation sink.
21 -Cheese opened commercially processed ready to eat food, located in the cold hold unit, and held more than 48 hours, was not marked with the date it was opened. -Some ready to eat items are not properly labeled and dated with the right items.
26 Employee medicines and personal hygiene products stored on shelf above preparation table in food preparation area, and were not stored to prevent the contamination of food, equipment or utensils. Employee removed medication.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible in cold hold units.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
36 -Visible physical evidence of rodent activity observed in the basement storage area, warewash and food preparation area. -Potential rodent harborage areas inside the food facility observed in the basement storage area due to unused equipment and clutter. Remove unused equipment.

Clean and sanitize items.
49 Hot water was not observed in one of four faucets at customer restroom hand wash sink. Person in Charge posted an "Out of Order" sign on faucet.
