13 -Observed vegetables being washed in three compartment sink in the kitchens ware wash area.
-Observed scooper handles submerged in food items stored in working bulk containers in the rear food prep
*Person in charge moved food items and utilized food prep sink instead.
*Person in charge removed single use scoop.
21 Potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the under counter refrigerators, was not date marked.
*Person in charge date marked food items.
35 The food in the bulk containers were not labeled with their common use names.
39 Moist wiping cloths observed lying on kitchen surface area and not stored in sanitizing solution
45 The following observations were noted:
- Damaged under-counter refrigerator in the kitchen area.
- Domestic reach-in and chest freezers observed in the kitchen.
- Two domestic microwave observed in the sushi area and prep area.
45 -Plastic crates observed used for food elevation in the prep area.
-Carboard lining utilized in the walk in cooler and on shelving units in the facility.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 -Ice accumulation observed in the food prep areas chest freezer.
-Food debris observed inside of reach in refrigeration units.
53 Mop observed not elevated for air-drying in the prep area.
53 Broom observed on the floor and not raised and elevated.
53 Food debris observed on direct floor surfaces throughout the food prep area.
54 - Dust accumulation observed on cover ventilation system in the kitchen /prep area.
- Hood filters have an accumulation of static grease.

PIC removed the ice, washed and sanitize the ice bin during inspection.
37 Boxes with oil observed stored on the floor in the cooking area.
PIC elevated the boxes with oil during inspection.
45 The following observations were noted:
- Damaged under-counter refrigerator in the kitchen area.
- Electrical box needs adjustment on the wall in the prep area.
- Domestic reach-in freezer observed in the kitchen.
- Two domestic microwave observed in the sushi area and prep area.
- Missing utility rack in the facility.
45 - Plastic crates observed used for food elevation in the prep area.
47 - Clean the interior of the under-counter refrigerator cooking area.
53 - Utility pipes penetration observed not properly sealed in food prep are.
- Missing ceiling tiles in top of the walk-in freezer.
53 Floor is in need of cleaning under the 3 compartment sink.
53 Mop observed in the bucket and not elevated for the air-drying in the prep area.
