1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. The person in charge had a Food Handler certificate only and did not demonstrate knowledge of food safety or proper HAACP plan execution. A Food Safety Certified Manager from CIBO across the hallwasy arrived and another Food Manager arrived shortly after. A food safety certified manager reviewed the process with the employee. *** Note: Food Safety Certified Managers can not be shared between two or more restaurants in the same shift. ***
8 No trash can at the hand sink in the prep/service area. A can was placed at the hand sink.
13 Raw fish wrapped in plastic was stored above and next to produce and ready to eat foods in the sushi prep refrigerator. The fish was placed in a bowl and moved to the lower shelf.
14 -There were no adequate stoppers for the 3 basin sink. They were provided during the inspection. Staff usually use the warewash machine.
27 The employee had not followed the HAACP Plan and documented the readings for the standards and the prepared sushi rice samples. He was not aware how to calibrate the pH meter. He was not observed following the HAACP Plan when asked to test the sushi rice. A Food Safety Certified Manager reviewed how to calibrate the pH meter. The HAACP Plan process was then conducted by the employee preparing sushi. The pH readings were documented in the log book. *** The readings, time and temperatures should be documented in the log book every time a batch of sushi rice is prepared. ***
45 -The 3 basin sink is leaking at the pipe below the sanitizer basin. -Stoppers were not working properly at the 3 basin sink. Stoppers were provided.
45 Cardboard covering the grease trap is not smooth and easily cleaned.
47 Residue observed on the grease trap outer surfaces.
53 Observations: -Residue on the floor perimeters in the cooking area around equipment. -Accumulated static dust on ceiling vent cover in the rear ware washing area.

45 The prep area hand sink was slow to drain initially. The person in charge unclogged the sink.
53 -Residue on the floor perimeters in the prep area under the equipment. -Static dust on ceiling vent cover in the rear ware washing area.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. There was no Philadelphia Food Manager present with a Food Safety Certificate. She arrived during the inspection.
8 The cooking area hand sink was blocked by a bucket of sanitizer. The bucket was removed.
8 No handwash reminder sign at the hand sinks in the shared bathrooms. They were provided.
14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 150 - 400 ppm as stated on the manufacturer's use directions. The employee manually dispensed the solution and it was tested at 200 ppm.
27 Food facility is not following the HAACP Plan that was approved for sushi preparation that was submitted and approved by the Department. The HAACP Plan, approval letter for the HAACP Plan, log for today for PH for sushi rice, distilled water, and pH meter were not readily available. Staff was not knowledgeable about a HAACP Plan. A pH meter and solutions were provided. However, there was no documentation for the approval of the HAACP Plan provided during the inspection. Also, distilled water was not readily available for proper testing. All Sushi prep must be stopped. An approved HAACP Plan with approval from The Health Department is required.
45 -The sanitizer dispenser is not working properly at the 3 basin sink. -Stoppers were not working properly at the 3 basin sink. Stoppers were provided.
53 -Excess water on the floor perimeters in the rear ware washing areas. -Static dust on surfaces in the rear ware washing area.
