1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. **Person in charge was instructed to remember the names of the 5 foodborne illnesses and not only the symptoms.
21 Bacon opened commercially processed ready to eat food, located in the walk-in cooler, and held more than 48 hours, was not marked with the date it was opened. **Person in charge dated the food.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in some under-the-counter/reach-in refrigeration units and reach-in freezers.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food in food prep area.

1 Food Safety Certified person was not present at the start of inspection. Food Safety Certified person arrived by end of inspection.
8 5-10 minute delay observed in hot water reaching peak demand. Hot water observed at a temperature of 80-81 degrees F. **PIC took corrective action and increased hot water heater during time of inspection. Hot water reached peak demand and was observed at a temperature of 100-110 degrees F during time of inspection.

1 Food Safety Certified person observed without a Philadelphia Certificate. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **An application for the Philadelphia Certificate was issued to PIC during time of inspection.
8 Paper towel dispenser empty at the handwashing sink in the rear food prep area.
8 The handwashing sink in the rear food prep area was blocked by an accumulation of cardboard boxes and not accessible at all times for employee use.
13 Used wiping cloth observed stored in drain of food prep sink. **PIC took corrective action and discarded used wiping cloth and properly washed, rinsed, and sanitized interior surfaces of food prep sink during time of inspection.
14 The following were observed: -Accumulation of food debris in wash section of 3 bay ware washing sink -Black residue and pink slime on the interior of the ice machine bin

36 The following were observed: -Visible physical evidence of rodent activity (mouse feces) around perimeters of floors and on shelving units (non-food contact) in front and rear food prep area Visible physical evidence of insect activity (flies) throughout the front and rear food prep area/ near any drains in the facility -Potential rodent harborage areas on the interior of the building perimeter in rear food prep area area due to accumulation of cardboard boxes
37 Food is not stored at least 6 inches above the floor in the walk-in freezer.
41 The following were observed: -Food dispensing utensil in the ice bins were stored in direct contact with food and not having handle stored above the top of the food -Serving utensils stored in stagnated water at a temperature of 74.8 degrees F. **During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in a container of water if the water is maintained at a temperature of at least 58°C (135°F) and the container is cleaned at frequency specified in § 46.712(d)(7).
