1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. Person in Charge arrived during time of inspection.
13 Several raw animal foods were stored above ready to eat foods in the open case and reach in refrigerator. Corrective action was taken and items were relocated.
14 Black residue, observed on the interior of the ice machine bin around side of bin. Corrective action was taken and ice was burned and surfaces were being cleaned and sanitized.
21 Packaged salads observed did not bear date marking.
27 Food facility packaging raw meat and various foods using reduced oxygen packaging without the required written procedures or HACCP plan.
35 Prepackaged salads is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
39 Moist wiping cloths observed lying on sandwich preparation area and not stored in sanitizing solution.
45 Unapproved vacuum sealer observed in food preparation area.
47 -Accumulation of ice on interior surfaces of chest freezer.
53 Debris observed behind refrigeration equipment in retail area.
62 Before engaging in an activity which requires a HACCP plan, a food facility applicant or operator shall submit a properly prepared HACCP plan to the Department for approval. Unapproved reduced oxygen packaging operation observed at establishment. A HACCP plan submission will be required before resuming operations.
