1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Application will be emailed with inspection report.
45 1. Non-NSF approved (domestic) equipment observed in the food prep area and basement - refrigeration/freezer unit and drying rack.
2. Leak observed at faucet fixture of handsink in kitchen
3. Unapproved galvanized mechanical exhaust hood observed in the food prep area. Variance request form received since 9/5/18.

14 Black residue, pink slime observed on the interior of the ice machine bin around the inside surfaces.
-Discontinue usage of ice machine immediately, unit must be cleaned and sanitized before future usage.
36 Visible physical evidence of rodent (mouse dropping) activity observed in the basement area.
-mouse dropping observed along the walls in the basement.

2. Unapproved galvanized mechanical exhaust hood observed in the food prep area. Variance request form received since 9/5/18.
3. Interior surfaces and racks in the reach-in refrigerator in the food prep area are rusting, cracked, and chipping.
4. Reach-in refrigerator in the food prep area has damaged door gaskets.
5. Defective refrigerator in the basement is used to store glassware.
6. The walk-in unit in the basement area was observed with a condensation leak and not in good repair.
47 -Interior surfaces of the domestic refrigerator-freezer unit in the basement are unclean.
-Interior of kitchen standing refrigeration unit is not clean to sight.
-Shelving in the bar service area are not clean to sight and touch.
51 Missing fan vent observed in both restrooms.
53 1. Floor through the facility is not clean to sight.
2. Ceiling and wall plaster observed chipping and peeling in the facility.
3. Unsealed and damaged concrete floors throughout the basement.
4. Seal all gaps observed in the basement and behind the bar service area.
54 Hood filters have an accumulation of static dust and grease.
