*Paper towel was provided.
45 Non NSF reach-in freezer (Frigidaire)observed in kitchen area.
-Observed damaged chest freezer doors and damage on the interior of units in basement
-Non NSF approved chest freezers observed in basement.
- Defective ice machine observed in basement.
47 The following observations were made:
- Food debris observed on bottom surface of reach-in freezer.
-Dust accumulation on vent covers in basement walk-in cooler.
-Food debris on floor surface of walk-in cooler.
-Heavy ice accumulation observed in all basement chest freezers.
-Walk-in cooler floor surface in need of repainting.
51 Dust accumulation observed on ceiling vents in restrooms.
53 -Dust accumulation on ceiling vents in kitchen area.
-Debris and dust observed along floor perimeter in 1st and 2nd floor bar area.
-Debris and dust observed on flooring surface in basement area.
-Damaged floor surface observed under rug in 1st floor bar area.
58 Alteration or construction began (Newly installed hood) prior to plan submission and approval. A completed Plan Review Application with proper plans, drawn to scale, showing all design features, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the, Office of Food Protection. -several unused equipment observed inside facility including: dishwasher and ice machine in basement, unused seafood steamer in kitchen
-galvinized hood present in kitchen
-leak at faucet of 3 compartment sink
-nozzle observed defective at prep sink in main kitchen
-rusted surfaces on bottom surfaces of reachin freezer unit in kitchen
47 -Clean inside the reach-in freezer located in the kitchen due to food debris and ice accumulation
-Heavy ice accumulation observed in all chest freezers inside facility
-fan guards on compressor inside walkin observed with dust accumulation
59 A 6 month window has been given at previous inspection on 5/11/2022 to replace the hood.
*observed hood not replaced at time of today's inspection
