1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. Note: PIC came at time of inspection.
6 Food employees observed donning single use gloves without a prior hand wash. Note: Employee must wash hands prior to wearing gloves.
8 Dishes was observed in the handwash sink. Note: Hand sink must not be used for anything other than hand washing. Tray was removed from the sink at time of inspection.
14 The can opener had an accumulation of food residue on the food contact surfaces. Note: Can opener was cleaned at time of inspection.
45 Walk-in cooler is used for drying storage.
47 Cleaning needed in the reach-in freezer unit in food prep, storage shelf and bar areas to remove accumulation of debris.

13 Uncovered food observed in the walk-in cooler. Note: All food must be covered prior to storing.
20 scallops and shrimps was held at 44.7 and 48.2 degrees F rather than 41 degrees F or below as required. Note: Food out of temperature was discarded.
21 Cut and cooked potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the walk-in cooler and cold holding units, was not date marked.
39 Moist wiping cloths observed lying on table and not stored in sanitizing solution.

41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients. Food dispensing utensil in the sauce observed in direct contact with food and not having handle stored above the top of the food.
47 Cleaning needed in all unit in food prep and bar areas to remove spills and accumulation of debris.
53 The following observations were noted: -Floor cleaning was needed under the spice rack due to accumulation of dust debris and mouse feces. -Floor cleaning was needed around the perimeters of the floor in the ware washing area -Floor cleaning needed under equipment in the bar area and in area at front entrance where wine are on display
54 Hood filters, light fixture and vent in walk-in cooler had accumulation of static dust and grease. Cleaning needed.

36 A glue trap was observed hanging from the ceiling in the food preparation area. The person in charge was informed to remove/relocate the glue trap to prevent contamination of food. The person in charge understood and removed/relocated the glue trap.
42 Glass plates and bowls were not properly inverted in the food preparation areas.
47 The following observations were noted: -Ice build-up in one chest freezer located in the storage area. -Rust around shelves in several refrigeration units. -Surface cleaning was needed around equipment in the food preparation areas and bar area due to food debris and residue.
53 Floor cleaning was needed under equipment in several storage areas, food preparation areas, and warewashing areas due to debris, grease and old mouse droppings.
53 Wall cleaning was needed in the food preparation area due to residue.
54 The hood filters located in the food preparation area were observed with an accumulation of static dust and grease.
