14 The can opener had an accumulation of food residue on the food contact surfaces. Corrective action : Person in charge removed, washed, rinsed and sanitized the can opener prior to completion of inspection.
14 Black residue, pink slime observed on the interior of the ice machine bin around basement food storage area. Corrective action : Person in charge dumped all ice from the ice machine bin, thoroughly cleaned interior of the ice machine bin prior to completion of inspection.
36 Gaps observed under facility's side exit doors (event service area), these outer openings in the food facility does not protect against the entry of insects, rodents, and other animals.
37 -Boxes of bulk food ingredients, boxed oils and other food related items are not stored at least 6 inches above the floor inside basement area storage cages, first floor storage closets and inside the walk-in cooler. -Uncovered butter inside the walk-in cooler.
45 -Cardboard linings inside one of the chest freezers in basement storage area. -Plastic crates being used for elevation of chest freezers in basement food storage area. -Plastic (milk) crates being used for elevation / storage inside walk-in cooler and basement area storage cages.
45 - Mesh filters installed under kitchen exhaust hood systems. - A non NSF / ANSI approved equipment :1 microwave oven (Toshiba), 3 reach-in freezers (Thomson, Maytag & Frigidaire), 6 chest freezers (Thomson, Maytag, Haier & Idylis) present in basement food storage area. - Two cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 -Static dust on walk-in cooler fan covers. -Clutter inside the walk-in cooler.
47 Static dust and grease accumulation present on filters under kitchen exhaust hood systems.
49 Vacuum breaker or backflow prevention device not observed on first floor area utility (mop) sink's faucet.
50 Condensate dripping from the basement area hot water heater tank into a plastic container, tubing not extended to approved indirect waste drain connection.
53 -Holes, peeling plaster, cracked and chipped ceiling present in basement food storage area. -Ceiling utility pipe penetrations in basement area are not properly sealed. -Floor utility pipe penetrations (underneath the warewashing machine) not properly sealed. -Rough surfaced, unsealed, cracked cement flooring in bar area (underneath counter).
53 -Debris present on along floor perimeters of food prep, basement storage and event space area (behind cooking equipment, warewahing machine & underneath the water main's backflow prevention device). -Debris present on steps leading into the basement. -Pooled water on floor in the basement storage area (underneath the mop sink). -Cluttered in some basement area storage cages.

53 Mops observed in utility buckets and not stored elevated to properly air dry.

50 -Condensate dripping from the basement area hot water heater tank into a plastic container, tubing not extended to approved indirect waste drain connection. -Observed overflowing grease trap in basement storage area.
