8 Paper towels were not available at one handwashing sink located in the food preparation area. The person in charge supplied paper towels.
14 Residue was observed on the interior of the ice machine bin around the upper rim. The person was informed to discard the current ice and discontinue the use of the ice machine until it is properly cleaned and sanitized.
36 Mouse droppings were observed in several areas. The following areas were noted: -On the floor under equipment in the warewashing area. -On the floor near the rear exit of the facility. -On the floor in the employee storage closet. -Around the perimeters of the floor under shelving in the basement storage area. -The lower surface of one under counter storage table in the food preparation area (no food contact).
37 Beverages were not being stored at least 6 inches above the floor in the basement beer cooler.
42 Plates were not properly inverted in the food preparation area.
45 One non operational under counter refrigerator was observed in the bar area.
47 The following observations were noted: -Rust around shelves in several refrigeration units. -The floor of one walk-in cooler located in the basement was in need of cleaning due to residue. -The fan guards in one walk-in cooler located in the basement was in need of cleaning due to dust accumulation. -One lower surface located in the food preparation area was in need of cleaning due to mouse droppings (no food contact).
53 Floor cleaning was needed under and around equipment in the food preparation area, warewashing area and basement storage area due to debris, grease and mousse droppings.
54 The hood filters were observed with an accumulation of static dust.

14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. Person in charge had the slicer cleaned during the time of inspection.
19 Reach in refrigerator temped at 55 degrees and cold prep temped at 44 degrees. Sanitarian advised that all food be moved to another location. Person in charge moved all items during the time of inspection.
42 Dishes observed not inverted with the food surface faced down.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
46 Food facility lacks sanitizer test strips to monitor the chlorine sanitizer concentration of the dishmachine.
47 -Ice accumulation observed in the chest freezers in the basement storage. -Dusty fan guards observed in the walk in cooler.
51 -Static dust accumulation observed on the vents in the restroom. -Sink observed needing to be resealed in the mens restroom.
53 Broom and dustpan were observed stored on the floor between uses; not hung on a utility rack as required.
53 - Tiles in the warewashing area and by the handsink closest to the kitchen entrance were in poor repair. - Caulking around the 3-compartment sink was in poor repair. - Pipes and wires penetrating the wall and ceiling are not sealed.
53 - Debris accumulations observed along the floor edges in the kitchen, bar, and basement storage areas. - Grease accumulations observed under cooking equipment in the kitchen area.
54 Reach in refrigerators observed without lights
54 Hood filters have an accumulation of static dust and grease.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. A person with a City of Philadelphia Food Safety Certificate arrived during the inspection.
41 - Common bowls or food containers observed without handles in direct contact with bulk food ingredients. - Utensils were observed being stored in stagnant water between uses.
45 - Gaskets on the under-counter refrigerator and the reach-in refrigerator were in poor repair. - Plastic crates were observed being used for elevation. - Concrete block legs, which are not smooth and easily cleanable, observed being used to support equipment.
46 Food facility lacks sanitizer test strips to monitor the chlorine sanitizer concentration of the dishmachine.
47 - Food debris observed on the floor of the walk-in cooler. - White debris accumulations were observed on the outside of the draft line in the beer walk-in cooler.
53 - Debris accumulations observed along the floor edges in the kitchen, bar, and basement storage areas. - Grease accumulations observed under cooking equipment in the kitchen area.
