1 Food safety manager lacks a Philadelphia Food Safety Certificate - application provided via email.
4 Cigarette butts found on the basement floor and under the warewash sink in the during the inspection.
6 Food employees observed donning single use gloves without a prior hand wash.
8 1. The handwash sinks in the bar and food prep areas were blocked by buckets and not accessible at all times for employee use - sanitizer buckets removed.
2. Soda holster drain observed draining into the handsink in the bar area.
3. Double basin, directly plumbed handsink in the warewash area is dually used as a food prep sink - facility lacks an indirectly plumbed handsink to properly wash produce and prepare foods.
8 Hand soap was not available at the handwash sink in the warewash area.
13 Raw foods were stored over ready to eat foods and produce in the cold prep unit refrigerator and walk in cooler - raw shrimp over crab mixture, and raw meet stored alongside produce.
14 1. Mouse droppings observed behind containers on the food prep table.
2. Black residue observed on the interior of the ice scoop holder in the basement.
20 Eggs, spinach, and cheese held between 47 - 52 degrees F, rather than 40 degrees F or below as required.
21 Sliced cheese located in the reach-in refrigerator was not marked with the date it was opened.
36 1. Visible physical evidence of rodent activity (mouse droppings) observed throughout the food facility.
2. Dead mouse observed on the basement floor.
37 A splashguard is lacking between the handwash sink and shelf where rolls and produce are stored in the food prep area.

3. Unused / defective equipment - commercial refrigerator (basement) and chest freezer (bar) used for dry storage, and stove (basement).
45 Raw wood, which is not smooth, easily cleanable, and non absorbent is used to as a holder for the ice scoop storage bin over the ice bin behind the bar.
46 Food facility lacks QAT sanitizer test strips to monitor sanitizer concentration.
47 1. Mouse droppings observed on shelving in areas (food prep, bar, and basement) throughout the facility, pallets used for storage in the basement, inside of fryer cabinets, inside defective chest freezer used for dry storage (bar), and on table surface where tabletop oven is stored.
47 Interior and exterior surfaces of equipment (fryers, refrigerator, refrigerator gaskets, cold prep unit, freezer) observed with grease and debris accumulation.
49 A backflow prevention device is lacking on the water line connected to the ice machine.
49 Leak observed from the main waterline near entry point in the basement.

2. Base coving is lacking along the floor-wall junctions in the basement/warewash area.

53 1. Ceiling and wall surfaces in the food prep area are unclean - Grease stained ceiling tiles observed around the exhaust hood, and debris on wall surfaces.
2. Floor surfaces in the food prep, basement and bar areas are unclean - food, debris, mouse droppings, and a dead mouse observed along the floor perimeters, behind and under equipment, on the windowsill, and basement stairs and ledge.
54 Inadequate lighting under the exhaust hood - light intensity in a food facility shall be at least 540 lux (50 foot candles) at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders or saws where employee safety is a factor.
54 Ventilation hood system is not preventing grease from collecting on walls, ceilings and fixtures.
