36 Gaps at the base of the front doors are outer openings in the food facility and it does not protect against the entry of insects, rodents, and other animals.

8 Soap and paper towels were not provided at both bars. An employee provided both items, while on site.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required at the bar. Management has agreed to manually wash and sanitize glassware, until machine is repaired or use the machine located on the R level of the building.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible inside refrigeration unit at food prep area.
