1 A verified Food safety certified person was not present at time of inspection. Corrective action was taken, Certified Food Safety person arrived before the conclusion of the inspection. **The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.** **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.**
8 Soap was not available at the handwash sink in the rear food prep area.
13 -Observed cooked chicken stored in plastic shopping bags(non-food grade bags) in the walk-in cooler on the pulley rack.
13 Observed metal cylinder cup without handle submerged in marshmallows stored in a food container in the rear food prep area.
14 Observed two files landing on clean inverted bulk food containers stored in drying rack in the warewash area.
21 Observed potentially hazardous ready to eat food(i.e. cooked chicken, sliced apples), prepared in the food facility and held for more than 48 hours, located in the walk-in cooler, was not date marked with the date prepared/opened.
26 Chemicals product observed stored above and on the same shelf of storage rack with equipment and single service articles in the warewash area.
35 Prepackaged desserts are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 Visible physical evidence of rodent (i.e. mouse dropping) /insect (flies) activity observed in the warewash behind drying rack and under warewash sink; under-counter of the front service areas and in the rear food prep area .
37 The following observations were noted: -Black trash bags used to cover raw whole chicken stored upright on metal skewers in the walk-in cooler. -Unwrapped raw whole chicken store on trash bags & cardboard on pulley prep table near the rear walk-in cooler. -Ready to eat food (cornbread) left uncovered on dry storage rack in the rear food prep area. -Boxes of food items stored on direct floor surface, not stored at least 6 inches above the floor in the walk-in freezer.
45 Cutting board on cold prep refrigerator unit observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
45 The following observations were noted: -External damage to top surface of Rotisserie Oven in the front food prep area. -All 3 beverage dispensing machines inoperable throughout food establishment. -Unused ice bin by the drive thru section.
46 Warewash sinks observed with food residue in compartments and not being cleaned and sanitized prior to use.
47 -Heavy ice accumulation observed on the floor and excess clutter in the walk-in freezer.
49 Observed a severe clogged floor drain under the food prep sink in the rear food prep area.
50 Observed uncovered 5 gallon buckets filled with grease stored on the floor in the warewash area.
53 Observed idle broom stored on direct floor surface, not elevated on a utility rack by the handwash sink.
53 Observed unsealed gaps around ceiling pipelines/wirelines in the food prep and warewash areas.
54 Observed food employees' belongings stored above or on the same shelf with single-use food serving items)
54 The following observations were noted: -Dust accumulation observed on ceiling vent covers in the food prep area. -Heavy accumulation of black dust and grease residue on vent shield of exhaust hood in the front food prep area.
