6 Food employees observed donning single use gloves without a prior hand wash. **Corrective action was taken, person in charge instructed food employee to discard the single use gloves and wash hands prior to putting on single use gloves to prep food at time of inspection.**
13 The following observations were noted: -Food left uncovered(i.e. cooked & uncooked chicken) inside the refrigeration units in the main food prep area. -Idle ice bins filled with ice left unlidded at beverage dispensing machines in the front service/food prep area. **Corrective action was taken, person in charge instructed food employees to cover food items which are stored and/or not being used.**
13 Observed food employee stacking baking pan with food(i.e. cooked chicken nuggets) without placing a protective covering between the food and external bottom surface of baking pan. **Corrective action was taken, person in charge discarding the exposed food at time of inspection.**
49 Observed a direct drain line installed under food prep sink. Food prep sink requires an indirect drain line. (c) Air gap requirement for backflow prevention. An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than 25 mm (1 inch).

37 Splash guard is needed between the soap dispenser and chicken prep area in rear.
45 Rear handsink in prep area is not properly sealed to the wall.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 Observations: -Residue under the racks of the reach-in refrigerators and freezer unit. -Debris and residue on floor surfaces in the walk-freezer unit.
53 Debris observed on floor surfaces in the ice cream station area and tools storage closet.
53 Observed maintenance tools in direct contact of the floor and not elevated or hung on a utility rack.
54 Hood filters have an accumulation of static dust.
