45 The following observations were made: - Unused exhaust hood system in the side storage area. - Non-NSF approved chest freezers (2), hand wash sink and drying racks observed in the ware wash area. -Unused ice cream freezer in retail area.

47 The following observations were made: - Dust accumulation on fan vent covers in walk-in coolers. - Rusted shelves observed in retail area.

53 The following observations were made: - Cracked floor tiles in ware wash area. - Dead flies observed on glue ribbon in ware wash and seating area. -Dead cockroaches observed on glue trap under ware wash sink.

1 The person in charge observed with a serve safe and not with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. * Food safety certificate link was sent via email.
8 Soap was not available at the hand wash sink in the food prep area and customers restroom.
8 No sign or poster posted at the hand wash sink in the kitchen area to remind food employees to wash their hands.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
36 Visible physical evidence of insect activity (House flies) observed in the rear storage area.
41 Spoon in coffee area observed stored in stagnant water.

51 -Dust accumulation observed on ceiling vent in employee restroom. -Loose ceiling vent observed in restroom area. - Customer restroom not cleaned to sight. - Door observed off the hinges.
53 The following observations were made: - Cracked floor tiles in ware wash area - Debris and dust on floor surface under slicer table. -Dead bugs, dust and debris observed in seating area.
53 Mops observed not hung to allow for proper air drying when not in use.
