21 Opened commercially processed ready to eat food (American cheese), located in the reach in refrigeration unit, and held more than 48 hours, was not marked with the date it was opened. *PIC provided a date mark at the time of inspection*
26 Chemicals were stored on the same shelf with equipment, on top of the ice machine. *These chemicals were relocated*
36 Gaps observed in front garage door (along the bottom). These outer openings in the food facility do not protect against the entry of insects, rodents, and other animals.
45 Equipment: -Non-NSF/ ANSI approved chest freezer -defective under-counter refrigeration unit being used for dry storage -crates being used to support chest freezer
45 Cardboard observed being used to line prep counters. Surfaces of equipment that are nonfood-contact surfaces, but are exposed to splash, spillage or other food soiling or that require frequent cleaning, shall be constructed of a corrosion-resistant, nonabsorbent and smooth material.
53 Holes observed in the ceiling and on walls.

45 - Non NSF approved utensils draining/drying rack present over the warewash (3-compartment) sink. - Observed a torn gasket on the chest freezer.
45 Observed cardboard lining the lower shelf of the prep table.
53 - Acoustic fissured ceiling tiles. - Water stained ceiling tiles. - Observed handwashing sink in the prep area and in the restroom not properly sealed to the wall.

8 The handwash sink did not have accessible single use towels, continuous towels, or air drying device. **Person in charge supplied the handwashing sink with paper towels at the time of the inspection.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 400 ppm, rather than 200 ppm as stated on the manufacturer's use directions. **Person in charge added additional water to the solution to bring the concentration to the correct level.
35 Working bulk food ingredient storage containers (salt & chicken seasoning) were not labeled with the common name of the food. **Person in charge labeled the containers at the time of the inspection.
45 - Non NSF approved utensils draining/drying rack present over the warewash (3-compartment) sink. - Observed domestic non ANSI/NSF ninja blender.
51 Observed restroom door not self closing.
