13 Observed a single-use food container without handle used as a scooper, submerged in the seasoned flour mix. **Corrective action was taken, person in charge removed, discarded and replaced the single-use food container with an actual scooper with handle at time of inspection.**

1 Observed food safety person with ServSafe Training Cert. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Health Sanitarian emailed Phila. Food Safety Certificate Application to person in charge.**
6 The following observations were noted: -Food employee is unable to properly was their hands; due to lack of water at any of the sinks on the mobile unit. -Food employees observed donning single use gloves without a prior hand wash.
8 Observed no available water at handwash sink at time of inspection.
13 Observed ready-to-eat food ( i.e. cheese) left unwrapped/uncovered in cold prep unit located on the mobile unit.
20 Raw chicken observed stored on prep surface in bulk working containers was held between 46.4 - 51.1 degrees F rather than 41 degrees F or below as required.
21 Observed opened commercially processed ready to eat food( i.e. cheese) located inside the cold prep unit, and held more than 48 hours, was not marked with the date it was opened.
35 Observed condiment bottles with unknown food item(i.e. dressing/sauce) not labeled with name of product.
40 **Observed whole produce( i.e. strawberries) stored inside cold-hold unit. Produce must be washed and sliced at approved support facility/commissary.**

6 Food employee could not properly wash hands, due to lack of hot water at 100 degrees F. minimum. *Hot water was restored.
36 Outer opening in the food facility does not protect against the entry of insects, rodents, and other animals: rear window/door is kept open without proper screening.
42 Air Drying rack lacks at 3-bay sink.
46 Food facility lacks quaternary ammonium sanitizer test strips to monitor sanitizer concentration.
49 Hot water faucet at handwashing sink is not in good repair. Hot water at or above 100 degrees F. was not observed at handwashing sink. *PIC corrected faucet and hot water issue.
