26 Observed spray can and spray bottle of chemical products placed/stored on the prep table in the food prep area.
49 The following observations were noted:
-A severely clogged urinal located in the toilet room.
-Severe water leak at pipeline under 3-compartment warewash sink in the food prep area.
-Water leak at faucet head at food prep sink.
51 Observed a severely clogged urinal located in the toilet room.
53 The following observations were noted:
-A small bird flew inside facility through the opening of the delivery rolling gate ramp area.
-Unused food prep, cooking/hot-hold equipment (i.e. mixer, two dual convention ovens) stored in the facility in the warewash/ food prep.
-Non food equipment/tools, numerous carts of stacked boxes and plastic crates stored in the both the bread storage area and the second storage area.
**(n) Removing unnecessary items and litter from the premises. The
premises of a food facility shall be free of items that are unnecessary
to the operation or maintenance of the facility, such as equipment that
is nonfunctional or no longer used, and litter.**
-Puddle of stagnant water located under the 3-compartment warewash sink; due to severe water leak.
53 The following observations were noted:
-Unfinished raw wood wall and entrance way frames are not sealed in the prep/warewash and storage areas, base coving is not provided.
-Openings in lower wall in the warewash/prep area.
-Concrete floors and wall contain damaged paint
58 **Plan review submission due to observation of installation of unfinished raw wood frames and wall surfaces to enclose/separate food prep/support facility area from the wholesaler/retail bread distribution area.
**Alteration or construction began prior to plan submission and approval.

The box of wrapped yeast was moved to the other refrigerator.

26 -Observed insecticides (a large bottle of Insect Spray) in the toilet room.
All pest control should be done by a licensed exterminator and be labeled safe for a food facility.
36 -Visible insect (flies) activity observed in the side storage area in the rear.
-Visible bird activity observed throughout the facility.
-Visible physical evidence of insect (live roach) activity observed in the food pre area.
36 -The open and unscreened pull up doors, outer openings in the food facility do not protect against the entry of insects, rodents, and other animals.
-Large gaps observed under other outer doors.
45 Observations:
-The True refrigerator condensation is leaking inside the unit and pooling on the bottom surface.
-The prep sink faucet is spraying.
-The microwave is not in good working order.

-Dirt and debris in the food prep sink.
-Residue, pooled and dripping water, and debris on inner surfaces of the reach in refrigerators.
-The walk in cooler is full making inspection difficult.
51 Toilet Room Observations:
-Static dust on surfaces
-Wet paint on the floor. Slipping hazard.
53 Observations:
-Flies observed around stacked boxes and plastic crates in the rear of the side storage area.
-Birds flying into the open doors on both sides of the facility.
-Some residue from bird droppings and debris on floor perimeters in the front of the side storage area near the large open garage door.
-Live roach on the floor near an opening in the lower wall in the prep area.
-Unused equipment stored in the facility in the warewashing/prep and adjoining right hand storage area. For example, four convection ovens, 2 display units, and other items.
-Numerous carts of stacked boxes and plastic crates stored in the right adjoining storage area.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
The person in charge does not have a Philadelphia Department of Health Food Manager Food Safety Certificate. Two employees have class certificates only.
4 Employees' beverage containers were observed in a food preparation area on the prep table.
8 There was no trash can or paper towels readily available at the hand sink in the warewash/prep area.
8 The warewash/prep area hand sink was dirty with a mop head stored in the basin.
9 Documentation of food distributors was not readily available for review.
26 -Observed insecticides (a large bottle of Home Defense Spray) in the toilet room.
-Observed a large bag of rat repellant not labeled by the manufacturer for food facility usage, in the front of the warewash/prep area.
The rat repellant was removed from the facility during the inspection.
26 -Chemicals (cleaning solution spray bottle) was stored above or on the same shelf with food, equipment, and/or single service articles in the reach in refrigerator in the prep/warewash area.
-Chemicals (motor oil and brake fluid) were stored above or on the same shelf with food, equipment, and/or single service articles in the warewash area.
35 -Prepackaged muffins, danishes, cinnamon rolls, and bags of rolls are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
-Prepackaged chocolate cookies are lacking the net weight, distributor address, and complete nutritional facts on the packaging.
36 -Visible physical evidence of rodent (mouse droppings) activity observed in the toilet room.
-Visible insect (fly infestation) activity observed in the retail, food prep, and storage areas.

-Birds landing on upper surfaces in the retail, front warewash/prep, and side storage areas.

-Unused equipment stored in the facility in the warewashing/prep and adjoining right hand storage area. For example, several convection ovens, 2 display units, and various other items.

-All sinks were observed heavily soiled.
9 Documentation for the manufacturer and/or distributor for all breads and other baked goods being sold was not readily available.
Provide invoices for all breads and baked goods sold in this facility.
11 A box of loose rolls, gloves and twist ties were observed in the facility.
The 3 basin sink and all hand wash sinks did not have water readily available.
The person in charge stated that the rolls were returned out of the package.
The rolls will be discarded.
26 Observed rodenticides (open poison pellets) not labeled by the manufacturer for food facility usage, in the toilet room area.
All pest control should be done by a licensed exterminator. All poison should be enclosed in a tamper resistant trap.
35 Prepackaged muffins and bags of rolls are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 Gaps at base of side sliding garage door, Exit door, front door, and open front sliding garage door (without air curtain or screen) are outer openings in the food facility that do not protect against the entry of insects, rodents, and other animals.
Potential rodent harborage areas inside the food facility observed in the closet in the warewashing area, due to miscellaneous items and equipment.
37 Boxes containing food are not stored at least 6 inches above the floor in the walk in freezer.
47 -Residue observed in the Delfield freezer.
-The 3 basin sink and the hand sink were heavily soiled containing static dust and debris in the basins.
-Static dust observed on surfaces throughout.
