1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
8 The food facility does not have a handwash sink conveniently located in the outdoor bar area. Note: Hand sink in outdoor bar has no hot water.
14 Black residue, pink slime observed on the interior of the ice machine bin around top and side. Note: Ice must not be used. ice was removed and machine was set up to clean.

2 Food facility does not have a Health Awareness Reporting Procedure for food employees. Note: A sample Health Reporting Policy was given to PIC at time of inspection.
6 Food employees observed donning single use gloves without a prior hand wash. Note: Employee must wash hand prior to wearing gloves.
7 A food employee was observed touching ready to eat food with his bare hands.
8 Old food residue and strainer was observed in the handwash sink in food prep area. Note: Hand wash sink must not be used for anything other than hand washing.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. City of Philadelphia Food Safety Certificate application was emailed.
8 - Soap was not available at the handwash sink in the kitchen area. Soap dispenser was refilled during the inspection. - Paper towel dispenser empty at the handwash sink in the kitchen area. Paper towel dispenser was refilled during the inspection. - The handwash sink in the bar area does not have single use towels, continuous towels, or air drying device. Paper towels were provided during the inspection.
13 Raw shell eggs were stored above ready to eat foods in the walk-in cooler. Eggs were relocated to the lower shelf of the storage rack during the inspection.

14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. The dish machine was primed and another cycle was run. The residual detected in the final sanitizer was measured at 50 ppm.
41 Common bowls or food containers observed without handles in direct contact with ice at the bar area ice bin.
