39 Moist wiping cloths observed lying on 3-compartment sink and not stored in sanitizing solution.
45 Non NSF / ANSI approved equipment :1 domestic microwave oven, 1 reach-in refrigeration / freezer unit (Insignia), 1 chest freezer (Holiday) "marked staff", 1 toaster oven (Oster), 1 flattop electric hot plate, 1 crockpot and a ceramic (porcelain) handwash sink present in the facility.
53 -Cracked and unfinished gypsum walls and ceilings present in the food prep area, ,hallway area and inside utility / storage closet. -Gaps in gypsum ceiling in hallway area. -Floor / wall junction base coving not present underneath the warewash (3-compartment) sink.
53 -Cleaning needed to the hallway carpeted floor. -Spider webs on walls & ceiling inside the utility /storage closet. -Debris present along floor perimeters of food prep area and inside the utility /storage closet. -Dead insects (crickets) on glue trap and debris on steps leading into the basement area.

26 Observed open rodent baits (powder form rodenticides) not labeled by the manufacturer for food facility usage, along floor perimeter of food prep area (underneath handwash sink and toaster oven table stand). Corrective action : Person in charge cleaned to remove the open rodent bait (powder form rodenticides) from floor perimeter of the food prep area prior to completion of inspection.
