6 Food employee observed donning single use gloves without a prior hand wash. Corrective action; Sanitarian explained the importance of handwashing in between tasks and before donning single use gloves, and the PIC instructed the food employee to wash their hands.
8 Observed a dish inside of the handwashing sink in the kitchen prep area. Corrective action; Person in charge removed the dish from the handwashing sink during the inspection.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay ware washing sink was in excess of 400 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. Corrective action; PIC diluted the solution with water to obtain the proper sanitizing concentration of 200ppm.
14 The can opener had an accumulation of food residue on the food contact surfaces. Corrective action; Sanitarian explained the importance of cleaning the can opener to avoid accumulations of food residue, and the PIC cleaned the can opener during the inspection.
39 Moist wiping cloths observed lying on equipment and not stored in sanitizing solution.
41 Knives and / or cleavers stored between table edges which are not easily cleaned and sanitized.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

- Observed the handwashing sink in the front coffee prep area slow to drain.
45 - Observed foil lining shelving surfaces in the kitchen prep area.

- Observed grease stains on the exterior surfaces of cooking equipment in the kitchen prep areas.
51 - Observed stains on the floor, ceiling, and wall surfaces in the employee restroom.
- Observed slight debris on the floor surfaces in the employee restroom.
53 - Observed stains on the ceiling and wall surfaces in the kitchen prep areas.
- Observed debris and stains on the floor surfaces and perimeters in the kitchen prep and storage areas.
53 Observed a dustpan and broom on the floor rather than properly hung when not in use.
53 - Observed gaps and holes in ceiling tiles in the kitchen prep and employee restroom areas.

**Dishes were removed at the time of the inspection.
8 Observed utensil washing sink provided with the handwashing aids (handwashing sign & soap) behind the counter area.
13 Observed the handle of the ice scoop in the ice.
**The handle was removed from the ice at the time of the inspection.

**Additional water was added to the solution to decrease the concentration.
26 Working containers in at the upper waste/utensil sink, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical.
36 Visible physical evidence of rodent activity (mouse droppings) observed in the lower prep area.
49 Observed faucet fixture on the 3-bay warewash sink leaking.

**New handwashing sign was placed at the handwashing sink at the time of the inspection.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required.
**Chlorine sanitizer was replaced with a new bottle and retested at the time of the inspection.
26 Working container (hand soap) in coffee area was not marked with the common name of the chemical.
**Person in charge replaced soap with a labeled soap dispenser at the time of the inspection.
34 Observed hot holding thermometer inside the under-counter refrigerator in the lower prep-kitchen.
**Person in charge placed an internal thermometer inside the refrigerator at the time of the inspection.
45 Observed missing vent cover in the lower floor restroom.
