41 The following were observed:
-Common bowls or food containers observed without handles in direct contact with bulk food ingredients
-Knives stored between table edges which are not easily cleaned and sanitized
45 The following were observed:
-Concrete block legs, which are not smooth and easily cleanable, being used to support equipment in basement area
-Raw wooden palates being used for elevation which are not smooth and easily cleanable
45 Leaking hot water heater in basement area.
45 Food contact surface of several cutting boards are not smooth, easily cleanable and resistant to pitting, cracking, or scratching.
47 The following were observed:
-Food debris on lower interior surfaces of all refrigeration units
-Ice accumulation on interior surfaces of chest freezers and reach-in refrigeration unit
-Grease accumulation on interior surfaces of cooking equipment

-Dead insects on several glue traps in basement area
**Removing dead or trapped birds, insects, rodents and other pests. Dead or trapped birds, insects, rodents and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests.
-Static dust, debris, and food debris throughout basement area and food prep area

6 A food employee was observed washing their hands at the food prep sink rather than the designated handwashing sink.
**PIC was instructed to have food employees wash their hands in the sinks designated for handwashing only during time of inspection.
8 Old food residue, dishes and utensils observed in the handwashing sink in the rear food prep area.
11 Potentially hazardous food located in under-the-counter refrigeration units with cold prep units attached were held at 45-50 degrees F rather than 41 degrees F or below as required.
13 Splash guard observed needed between handwashing sink and food prep sink.
14 The following were observed:
-Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours
-The can opener had an accumulation of food residue on the food contact surfaces

1 Food Safety Certified person did not demonstrate proper knowledge pertaining to sanitization. PIC observed putting sanitizer in the wash section of the 3 bay ware-washing sink.
**PIC was instructed on the correct way to set up 3 bay ware-washing sink during time of inspection.
8 The following were observed:
-Soap was not available at the handwashing sink in the front food prep area
-Paper towel dispenser empty at the handwashing sinks in the front and rear of the food prep area
**PIC took corrective action and provided soap and paper towel during time of inspection.
8 The following were observed:
-The handwashing sink in the front food prep area was blocked by a chair and not accessible at all times for employee use
-Old food residue, dishes and utensils observed in the handwashing sink in the rear food prep area
**PIC took corrective action and moved chair and soiled dishware to designated areas during time of inspection.
11 Potentially hazardous food located in reach-in refrigeration units and under-the-counter refrigeration units with cold prep units attached were held at 50-60 degrees F rather than 41 degrees F or below as required.
13 Pre-cooked chicken observed being prepped in the rinse section of the 3 bay ware-washing sink and not in the food prep sink as required.
**PIC took corrective action and discarded pre-cooked chicken during time of inspection.
14 The following were observed:
-Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
-The can opener had an accumulation of food residue on the food contact surfaces.
**PIC was instructed to wash, rinse, and sanitize food contact surfaces of food equipment every 4 hours as required during time of inspection.
20 Potentially hazardous food located in reach-in refrigeration units and under-the-counter refrigeration units with cold prep units attached were held at 50-60 degrees F rather than 41 degrees F or below as required.
21 Potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the reach-in refrigeration units, were not date marked.

-Broken rear door
