53 - Unsealed gaps around hood vents and wall surfaces in the rear prep area and bakery area.
53 -Debris on the floor in the rear prep area.
58 Plans for the rear prep area received on 1/31/2022. No grilling operation until the plans are evaluated.

4 An open employee's beverage container was observed in a cold prep unit in the food preparation area.
21 Bulk food containers (cheese) opened commercially processed ready to eat food, located in the reach-in units and cold prep units, and held more than 48 hours, was not marked with the date it was opened.
26 Observed rodenticides (mouse bait) not labeled by the manufacturer for food facility usage, in the food area behind a chest freezer.
37 Beverage observed stored on the floor and not elevated at least 6 inches above the floor in the storage area.
41 Knives and / or cleavers stored between table edges which are not easily cleaned and sanitized. PIC removed the knives from the table wedged and washed, rinsed, and sanitized during inspection.
45 The following observations were noted: Aluminum foils wrapped over wooden pallet in storage area. Cardboard being used as lining in reach-in fridge. Plastic crates observed for elevation in the food prep area.

14 - The can opener had an accumulation of food residue on the food contact surfaces.
14 The following observations were noted: -The food-contact surfaces of equipment and utensils did not reach 120 -140 degrees F during the sanitizing cycle of the low temperature dishwasher as evidenced by the failure of the temperature indicator. Temperature was measured at 90 degrees F during sanitizing cycle. *Sanitarian advised the person in charge to use temporarily the 3 bay sink to washed, rinsed, and sanitized the utensils and equipment until the dishwasher will be fixed. -Sanitizer dispenser was empty at the 3-basin sink.  PIC placed sanitizer in the dispenser during inspection.
21 - Several food items observed without the dates in the under-counter refrigerator and reach-in refrigerator in prep area. *Sanitarian advice the person in charge to provide dates for the food.
36 -Visible physical evidence of rodent (mouse droppings) observed in the storage area along the floor perimeters.
45 Aluminum foils wrapped over lower shelving of the tables in rear prep area.
45 Non NSF / ANSI microwave "Solve" observed in the prep area.
45 Non-food grade shopping bags are used for food storage in the reach-in freezer rear kitchen area.
48 Water not under pressure at the 3 bay-sink rear prep area.
53 - Fresh mouse dropping and debris observed on the floor in the storage area.
53 Utility pipe penetration observed not properly sealed in food prep areas.
54 Dusty cover fans observed in the prep area.

1 The food safety certified handler was not present at start of inspection, she arrived prior to completion of the inspection.
8 Sponges observed in the handwash sink in the prep area. PIC removed the sponges from the handwash sink during inspection.
21 No dates observed for prepared food in the facility (reach-in refrigerators, closed display cases and under-counter refrigerators) that are held for more than 48 hours.
37 Beverages and bags with flowers observed stored on the floor in the storage area and prep area not elevated at lest 6 inches above the floor.
45 The following observations were noted: - One broke handle observed at the reach-in refrigerator in the prep area. - One domestic microwave "Solve", one domestic reach-in refrigerator and two domestic chest freezers observed in the facility. - The dishwasher observed out of service.

53 Gaps observed in the ceiling in the tandoori prep area.
59 Plan review application has to be submitted for the shaorwama spinning machine with ventilation hood. Corrective action is required to eliminate these violations.
