13 - Missing a food prep sink with indirect waste drain connection and catch basin in the facility. Sanitarian observed people from the program eating grapes. Picture attached to the e-mail on 4/30/2025.
14 - Missing the 3 compartment sink. Regular cups observed being use in the facility. ** Utensils (cups) and food-contact surfaces of equipment shall be sanitized before use after cleaning.
20 - Food (one container with shrimp) was held at 101 degrees F rather than 41 degrees F or below as required. PIC discarded the shrimp during inspection.
21 No dates observed on food containers with cooked collard greens, beans, and a bag with dry fish.
39 Wet rugs observed on the counter and not stored in a bucket with sanitizer solution in the kitchenettes 1st and 2nd floor.
45 Several hand wash sink not seal to the walls in the restroom area: basement, 1st floor, and 2nd floor.
49 -An approved indirect waste drain connection with a 1 inch minimum air gap lacks in the facility.
51 - Missing the trash can with lids in several restroom in the facility.
53 - Peeling paint observed on the walls in the woman's restroom area / first floor. - Gaps observed around the utility pipe in the second floor/activity room. - Stains observed on the walls in the 2nd floor kitchenette. - Detached ceiling tiles in several restrooms in the facility.
53 Dust accumulation and debris observed on the floor in the therapy room.
54 - Static dust observed on the ventilation cover 2nd floor restroom.

51 The following observations were noted: - Dust accumulation observed on the vent cover bathroom/basement area. - Gaps observed around the sink in one of the bathroom / basement area. - The hand wash sink observed not seal to the wall/basement area.
53 - Water ceiling stains observed in one classroom second floor. - Gaps observed around the utility pipe in the kitchenette / first floor.
