1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
Corrective action; PIC arrived during the inspection.
1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
Food employee at time of inspection has valid ServSafe only. Corrective action; Sanitarian emailed PIC copy of Philadelphia Food Safety Certificate application.
4 - An open employee's beverage container was observed in a food preparation area. Corrective action; PIC instructed food employee to discard open beverage container during the inspection.
6 - Observed several food employees not following proper handwashing procedure. Proper handwashing aides (soap, paper towels) were not available at the handwashing sink in the kitchen. PIC replenished handwashing aides at handwashing sink in the kitchen during the inspection. Observed food employees continuing not following proper handwashing procedure (i.g. not washing hands between tasks, using soap and warm water) during the inspection after sanitarian advisement.
8 - Observed spoons and equipment in the handwashing sink at the front coffee prep area. Corrective action; Food employee removed the items from the handwashing sink during the inspection.

8 - No sign or poster posted at the handwashing sink in the kitchen area to remind food employees to wash their hands. Corrective action; PIC placed an employee handwashing reminder sign at the handwashing sink during the inspection.
- Soap was not available at the handwashing sink in the kitchen area. Corrective action; PIC placed a bottle of handwashing soap at the handwashing sink during the inspection.
- The handwashing sink in the kitchen area does not have single use towels, continuous towels, or air drying device. Corrective action; PIC replenished the paper towels at the handwashing sink in the kitchen area during the inspection.
13 - Observed unshelled eggs and raw chicken stored above open containers of RTE food items and deli meat in the reach-in refrigerator. Sanitarian advised PIC to move the food items to an appropriate shelf during the inspection.
- Observed several uncovered RTE food items inside of the reach-in refrigerator and under-counter refrigerator. Sanitarian advised PIC to place proper coverings on food items during the inspection.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay ware washing sink was 100 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. Corrective action; PIC added more sanitizer to the solution to derive the proper concentration of 300ppm during the inspection.
14 Pink slime observed on the interior of the ice machine bin. Corrective action; PIC agreed to discontinue use of the ice and ice machine until it can be properly cleaned and sanitized. PIC began to discard ice during the inspection.
21 Opened commercially processed ready to eat food (deli meat), located in the reach-in refrigerator, was not marked with the date it was opened. Sanitarian advised PIC to place a date on the food items during the inspection.
33 Chicken being thawed using an unapproved method. Food observed thawing at room temperature inside of the prep sink (no running water).
37 Beverages are not stored at least 6 inches above the floor in the kitchen/storage areas.
39 Moist wiping cloths observed lying on equipment and not stored in sanitizing solution.
44 Single use gloves being used for multiple tasks without being changed, or not being discarded when damaged or soiled.
47 - Observed grease and food debris accumulations on the shelving surfaces underneath the cooking equipment in the kitchen.
48 - Observed hot water slow to generate at the 3-bay sink, restroom, and kitchen handwashing sink during the inspection. Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food facility.
53 - Observed maintenance tools idle on the floors rather than properly hung when not in use.
53 - Observed clutter, dust, and debris on the floor perimeters at the front coffee prep area.
- Observed dust and debris on the floor perimeters and in between equipment in the kitchen, storage closet, rear ware washing area, and rear storage areas.

36 The base of the rear door is an outer opening in the food facility and it does not protect against the entry of insects, rodents, and other animals.
38 Food employee observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
39 Moist wiping cloths observed lying on equipment and not stored in sanitizing solution.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
45 The sink stopping operation of the 3 compartment sink is defective.
A household chest freezer observed in the storage/office room.
45 Milk crates are used for item storage, instead of approved shelving in the prep and storage area.
47 General counter, shelf, and cabinet surface cleaning is needed near the coffee station in the cafe area due to food debris.
51 Toilet room doors are not self-closing (3 of 4).
52 Dumpster lids are left open in the rear refuse area.
Trash and debris observed on the ground of the rear refuse area.
53 General floor and wall surface cleaning is needed in several areas due to dust and debris:
