8 Observed a food employee rinsing off cooking utensils in the handwash sink in the prep area. Corrective Action: Directed food employee to utilize the three basin sink when washing, rinsing, and sanitizing food equipment and utensils.
8 Paper towels observed not present at the handwash sink in the restroom. ** Advised to supply paper towels at the handwash sinks at all times.
11 -Observed an open can of mushrooms in the lower compartment of the closed display case not covered or protected from contamination. ** Advised to cover all food products stored in containers to prevent contamination. -Observed dented canned items and expired baby food products stored on retail and POS shelves. ** Advised to regularly monitor the canned items for dents and baby food products for upcoming expiration dates.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. ** Advised to wash, rinse, and sanitize the deli slicer at least every 4 hours.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 50 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions. Corrective Action: The sanitizer was drained and remade and it retested at 200 ppm. ** Advised to regularly monitor the sanitizer levels and to replace the sanitizer water at least every 4 hours.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the prep area. -Observed WD-40 stored on top of the closed display case. -Observed chemical sanitizer and soaps stored on the drying rack above the three basin sink. ** Advised to promote chemical/food separation storage by storing chemicals beneath food products, equipment, and containers at all times.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. ** The Philadelphia Food Safety Certificate application was emailed.
1 The Person in Charge described not washing, rinsing, and sanitizing food contact surfaces (deli slicer, cutting board, food prep sink, and cooking utensils) at least every four hours. ** Advised for all food contact surfaces and utensils to be washed, rinsed, and sanitized every four hours.
11 Observed burger patties and chicken nuggets stored in the reach-in freezer not covered or protected from contamination.
13 Observed raw shell eggs stored above ready-to-eat sauces in the bottom portion of the cold prep unit. ** The eggs were relocated.
