6 A food employee was observed washing their hands at the rinse basin of the warewashing sink rather than the designated handwash sink.
14 -The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink in the rear was 0 to 150 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Person in charge stated that the sanitizer has been in the sink overnight, and it was not being used today. Corrective action: Sink emptied. -The quaternary ammonium concentration in the sanitizing solution currently in use at the 3-bay warewash sink in the front of the facility was 0 to 150 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Corrective action: Fresh sanitizer solution was prepared.
18 Potentially hazardous food (potatoes), was not cooled from 135 degrees F to 70 degrees F in 2 hours and/or from 135 degrees F to 41 degrees F within 6 hours, after preparation. Corrective action: Potatoes were placed in the freezer for rapid cooling shortly before reaching 6 hour point.
20 Potentially hazard food was held at 44 degrees F rather than 41 degrees F or below as required. Reminded the person in charge (PIC) to use deeper containers for the tomatoes. Corrective action: Tomatoes were placed in a deeper container during the inspection, and temperature decreased to 39 degrees F.
22 Time in lieu of temperature being used in the food facility to control ready to eat and raw potentially hazardous foods without written procedures or documentation to verify disposition of food. (shell eggs and butter blend labeled, "Keep Refrigerated". Fried onions and cooked potatoes stored on shelf above the flat grill were observed being placed in the trash during the time that Sanitarian was conducting handwashing.) Corrective action: Eggs at 47 degrees F were labeled with the date and disposal time.
26 Rodent bait station with exposed bag of rodent bait was observed stored behind the coffee machine on the table in the food prep area. Corrective action: Bait station was removed.
36 Visible physical evidence of rodent activity observed in the facility. Mouse feces was observed in the following areas: -Floor behind the reach-in refrigerators in the front food prep area. -Floor in the garage area where reach-in refrigerator and freezers are in use. -Floor beneath the table in the dining area used for storage.
37 Food is not stored at least 6 inches above the floor. (boxes of potatoes)
45 -Garbage grinder switch does not work. Grinder is plugged in to power it. -Chest freezer is not NSF/ANSI approved. (i.e. Frigidaire) -Reach-in freezer is not NSF approved. -Perforated pan used as drying rack has large cracks in it. -Tape observed on top edges of ice machine. -Refrigerator gaskets are in disrepair. (i.e. under-the-counter refrigerator)
45 Food observed stored in grocery bags rather than food-grade bags.
47 Cleaning issues: -Exterior of containers observed soiled in garage area. -Accumulation of dust observed on fan cover in the rear food prep/warewashing area. -Debris observed at the bottom of the reach-in freezers in garage. -Rust observed on racks in under-the-counter refrigerator. -Refrigerator gaskets and interiors need cleaning. -Exterior of refrigerator in the garage area needs cleaning.

7 Food employees were observed touching ready to eat food with bare hands. The following were observed: -Person in charge picking up and cutting a sandwich with bare hands. -Food employee picking up slices of cheese and placing it on omelet with bare hands.
13 -Raw animal foods (scrapple, bacon, etc.) were observed stored above ready to eat foods (milk, butter, cheese, etc.). Corrective action: Food rearranged during the inspection. -Uncovered sugar at coffee prep station and uncovered bacon in refrigerator with a dusty fan cover. -Uncovered French toast batter observed in under-the-counter refrigerator with a dusty fan cover and stored below a rusty chipping rack. Discarded.
13 -Heavy build-up observed on waffle iron plates. -Cooked bacon observed stored in cardboard egg trays in the refrigerator. Corrective action: Multiple requests made for bacon to be discarded. -Greasy cardboard observed covering French toast in reach-in refrigerator. Corrective action: Person in charge instructed to cover the French toast with something clean and made for storing food.
22 Time in lieu of temperature being used in the food facility to control ready to eat and raw potentially hazardous foods without written procedures or documentation to verify disposition of food. (shell eggs, cooked potatoes, cooked sausage, cooked onions and butter blend labeled, "Keep Refrigerated".) Corrective action: Potatoes and onions discarded. Eggs placed in the refrigerator and sausage removed from above the grill for proper cooling for refrigeration.
36 Visible physical evidence of rodent activity observed in the facility. Mouse feces was observed in the following areas: -Small ledge behind and above shelf holding toaster in the food prep area. -Floor, cider blocks and shelving holding containers of food in the attached garage.
37 Food observed stored in attached unfinished garage. Garage is not approved for food storage.
43 Plastic forks are not stored so that contamination of food- and lip-contact surfaces is prevented.
43 Single use items are being reused for further food storage and / or preparation. The following was observed: -Cooked bacon stored in egg trays. -Greasy cardboard covering French toast sticks.

(i.e. Frigidaire) -Reach-in freezer is not NSF approved. -Pan used as drying rack has large cracks in it. -Crates used to elevate onions. -Tape observed on top edges of ice machine. -Refrigerator gaskets are in disrepair. (i.e. under-the-counter refrigerator)
45 Garage shelving used for storing containers of mayo, spices, etcetera, are not smooth, non-absorbent and easy to clean.
47 Cleaning issues: -Exterior of bulk container observed soiled. -Accumulation of grease and some food debris observed beneath the flat grill. -Accumulation of dust observed on fan cover in under-the-counter refrigerator. -Debris observed at the bottom of the reach-in freezer. -Rust observed on racks in under-the-counter refrigerator. -Dusty fan cover observed in rear prep area. Cleaning is required. -Clean and sanitize the counter and shelf behind the toaster where a few mouse droppings were observed. -Refrigerator gaskets are not clean. (i.e. under-the-counter refrigerator)

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Food safety certified person was not on site at the start of the inspection. Corrective action: Food safety certified person arrived during the inspection.

Multiple requests were made for a wash solution to be prepared in a basin for washing the slicer and can opener rather than the small container observed. Note: It is the duty of the person in charge to monitor warewashing solutions throughout the day.

45 -Cardboard box observed being reused for utensil storage. -Cardboard with duct tape observed dividing the plastic utensils. Note: Surfaces should be durable, non-absorbent and easy to clean.
45 -Garbage grinder observed on a sink with an open drain. A garbage grinder may only be attached to direct drainage. -Kick-plate is missing from ice maker in the front prep area. -Top of the cold prep refrigerator is not keeping food cold. Sanitarian suggested deeper containers with lids and testing food temperatures with a sanitized probe thermometer. -Leg observed missing from handwashing sink in the front food prep area. -(Ref. 46.501) Drying rack is not NSF approved.
46 Multiple packages of expired or incorrect test strips were observed. Corrective action: Expired strips were discarded. Note: Quaternary test strips are needed for quaternary ammonia tablets, and chlorine test strips are needed for chlorine/bleach approved for dishes.
