26 Chemicals(i.e. 2 air fresheners can sprays) observed stored on the same shelf with food related items in the clerk area.
**Corrective action was taken, store clerk related air fresheners to the bottom shelf at time of inspection.**
45 Observed cardboard used as shelf liners in the reach-in refrigerator in the retail area.
45 Observed a non-NSF/ANSI microwave stored in the clerk area.
48 Observed hot water delay at handwash sink in the toilet room.
49 No mop sink observed in the facility.
51 The following observations were noted in the toilet room:
-Handwash sink not clean to sight.
-Dust accumulation on ceiling vent.
53 The following observations were noted:
-Water stain ceiling tiles in the retail area.
-Ceiling tiles not secured in place in the retail area.
53 Wet mop observed in a bucket and not hung or elevated for effective drying.
53 Observed two large cardboards used a floor liners at the entrance doorway of the retail area.

36 Gap observed between the front door where the outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
45 Raw wooden wall panel observed inside the walk-in cooler that is not resistant to peeling and chipping.
47 The following observations were noted:
-Ice accumulation observed inside ice cream/water ice freezer.
-Light dust accumulation observed on shelves in the retail area.
49 No mop sink observed in the facility.
53 Wet mop observed in a bucket and not hung or elevated for effective drying.
53 Residue and debris observed on floor service in the hallway adjacent to the walk-in cooler.
54 Dust accumulation observed on ceiling vent covers.
54 Two lights observed out in the walk-in cooler.
