33 Prep sink observed without cold water. Tomatoes and frozen guacamole were observed thawing in the prep sink at the time of inspection. When thawing frozen foods with running water, the water must be no higher than 70 degrees F.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Department certified person arrived during inspection.
8 Handwashing sink behind the bar had a strainer in it. **Person in charge removed strainer during inspection. Handwashing sink only to be used for handwashing.
13 Observed raw chicken stored above potentially hazardous ready to eat foods (chicken wings) in the bottom of the cold prep fridge.
14 Food contact surfaces: - The potato/french fry press has an accumulation of old food debris/residue. - Slight accumulation of black residue on the interior baffle of the ice machine in the basement.
17 Foods in the hot holding unit, which were cooled, were not reheated to 165 degrees F for 15 seconds as required. **Person in charge was instructed to heat foods before putting them in the hot holding unit; reheated during inspection.

14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature kitchen sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. * Person in charge set up the 3-compartment sink to wash, rinse, and sanitize all multi0use equipment/food contact surfaces.
36 Visible physical evidence of rodent / insect activity observed: - Fruit flies in and around the dump sink behind the bar. - Flies in the kitchen/ warewashing area. - Older rat droppings on floor under the 1st fl. bar ice well, in the basement closet (around sump pump), and in the wall/floor junctures in kitchen (East-side).
