1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
- Food safety certified person arrived after inspection was complete.
4 Food Employees eating in *the food prep area as evidenced by observed partially consumed food in the *food prep area.
-Observed platters of food for the employees stored on the sandwich prep counter.
6 Food employees observed donning single use gloves without a prior hand wash.
8 -No paper towel, soap or handwash reminder sign observed at the handwash sink in the toilet room area.
-No handwash reminder sign at the handwashing in the food prep area.
8 The food facility does not have a handwash sink conveniently located in the retail coffee station* area.
-Either remove coffee station or add a handwashing sink in that area.
13 Observed missing ceiling tile above the sandwich cold prep unit.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
-Not able to fully see if slicer was clean due to food employees inability to open slicer.
21 * Deli meats opened commercially processed ready to eat food, located in the *closed display case, and held more than 48 hours, was not marked with the date it was opened.
36 Visible physical evidence of insect activity (live German cockroaches) observed in the *food prep area.
-Observed live roaches harboring under the food scale.
45 Observed a shallow basin sink installed at the food prep sink.
-Provide a deep basin sink.
45 Observed cardboard lined shelving in the reach-in refrigerator.
45 Observed domestic "GE" microwave in the food prep area.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 Food debris observed in the bottom of the cold prep refrigerator.
51 No light observed in the toilet room.
53 Porous ceiling tiles observed in the food prep area.
53 -Some debris observed on the floor perimeters.
-Water damaged ceiling tiles observed in the retail area.
-Missing ceiling tile above the cold prep unit.
54 1 light bulb out under the exhaust hood.
58 Alteration or construction began prior to plan submission and approval. A completed Plan Review Application with proper plans, drawn to scale, showing all design features, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the, Office of Food Protection. 8 No paper towel, soap or handwash reminder sign observed at the handwash sink in the toilet room area.

-Observed a bottle of Raid.

-The drinks were discarded during the time of inspection.
8 Paper towel dispenser empty at the handwash sink in the toilet room area.
11 Expired Gerber baby food observed.
-These items were discarded during the time of inspection.
13 No food prep sink available to wash fresh produce used in hoagies.
Provide a sink with a 1-inch air gap or apply for a variance.

45 Cardboard lining shelving in the reach-in refrigerator.
45 At time of inspection the following non NSF/ANSI approved equipment was observed:
- garbage grinder installed on a shallow basin wall mounted handsink. Install grinder on a deep basin free standing NSF/ANSI approved sink with 4 legs.
- domestic deep freezer in rear prep area
- Domestic toaster
-Domestic microwave
-Kick plate missing on the display case.
-Exposed insulation on chest freezer.
-Garbage grinder non operational.
47 Food debris observed in the bottom of the cold prep refrigerator.
49 Observed a leaky pipe under the hadnwashing sink in the food prep area.
53 -Observed grease and food debris on the floor under/behind the cooking equipment.
-Water damaged ceiling tiles observed in the retail area.
