1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. City of Philadelphia Food Safety Certificate.

4 Observed an employee's partially consumed meal in prep area (basement) at time of inspection.
8 Handsink in prep kitchen on opposite side of warewash sink (1st floor) observed not operable at time of inspection.
8 No sign or poster posted at the handwash sink in the front wait station area to remind food employees to wash their hands.
8 Soap was not available at the handwash sink in the front bar area.
12 Records of identification. The identity of the source of shellstock that are sold or served shall be maintained by retaining shellstock tags or labels for 90-calendar days from the date the container is emptied by the following methods:
(1) Using an approved recordkeeping system that keeps the tags or labels in chronological order correlated to the date when, or dates during which, the shellstock are sold or served.
-Shellstock tags for mussels were observed not stored in chronological order at time of inspection
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required at front bar undercounter dishwasher.
26 Containers of poisonous or toxic materials and personal care items shall bear a legible manufacturer’s label.

34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible inside refrigeration equipment.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food (sugar).
37 During preparation, unpackaged food shall be protected from environmental sources of contamination, such as outside dust, construction or renovation debris, or ventilation dust.

41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients (plastic containers without handle, observed used as a scoop).

41 Knives and / or cleavers stored between edges of food prep refrigerator (1st floor) which are not easily cleaned and sanitized.
45 Food contact surface of cardboard lining shelving units inside walkin is not smooth, easily cleanable and resistant to pitting, cracking, or scratching.

-Observed defective air curtains in walkin unit at time of inspection.
-Observed missing handle for walkin door (near restroom) in basement.
-Leaks observed at faucet fixtures on several service sinks throughout facility.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration for quatenary sanitizer.
47 -Heavy ice accumulation observed in walkin freezer, so much ice observed in excess that walkin freezer door cannot close.
-Chest freezers at time of inspection.
53 Observed some ceiling areas missing in basement prep area and warewash area of basement.
54 Inadequate lighting observed in front of stove top/prep area in basement kitchen area
