14 The concentration in the wiping cloth bucket was greater than 100 ppm. Note: The person in charge diluted the chlorine concentration to less than 100 ppm.
22 Time stamps at the food line were inaccurate. Note: The person in charge was instructed to remove the food or provide accurate time stamps.
39 Moist wiping cloths observed lying on sinks and counters and not stored in sanitizing solution.

47 The area and counter around the out-of-service soda fountain in the seating room were unclean to sight and touch.

1 Certified person did not have a city certificate. Application was given to the person in charge. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
6 Food Employee did not follow proper handwashing procedure. Employee was observed using hand sanitizer instead of soap and/or warm water not used. Employees were instructed to wash hands.
13 A wire was dangling in the ice in the ice machine. Wire was removed.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods (tomatoes, cheese) without written procedures or documentation to verify disposition of food. Time logs were created.
41 Containers in the prep area were not inverted to prevent contamination from dust and debris.
45 Broken door to the bottom compartment of the McFlurry machine.
46 Sanitizer test strips to monitor sanitizer concentration was not observed.
47 Observations: - Food debris observed inside shelves in prep area - Food debris accumulation on bottom surface of walk-in freezer - Grease accumulation observed under and behind hot hold unit. - Dirty glue boards, residue, and grime observed in cabinets under the soda machine in the customer area.
53 Unsealed utility lines observed in the rear area.
53 The following observations were made: - Food debris under equipment throughout prep area. -Debris, cups, and lids under beverage dispensers behind cash register area. - Dust accumulation on vents in walk-in cooler. * General cleaning is needed.
54 Hood filters have an accumulation of static dust and grease. Dust accumulation observed on vent covers throughout the facility.
