8 Dishes and coffee utensils observed in the handwash sink in the front service/ coffee station area. *PIC removed these at the time of inspection*
8 No soap or paper towels were observed at the handwash sink in the rear prep area. *PIC provided soap and paper towels at the time of inspection*
35 Prepackaged foods (pastries) are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 Visible physical evidence of rodent activity observed: -mouse droppings observed in the front coffee station area (underneath the sink) -mouse droppings observed in the rear prep area along floor perimeters -mouse droppings observed in the customer restroom underneath sink area
36 Gap observed to the bottom right of the rear entry/ exit door (toward trash area). Gaps also observed in front entry door- These outer openings in the food facility do not protect against the entry of insects, rodents, and other animals.
45 Equipment: -non-NSF/ANSI approved mini reach-in refrigerator observed -observed missing handle on the prep cooler lid. -rear 3-basin sink and handwash sink not properly sealed to the wall -observed many unused equipment units in rear storage area

8 The handwashing sink in the back kitchen restroom area was blocked by a mop bucket and not accesible at all times for employee use. **Person in charge had the mop bucket relocated at the time of the inspection.
45 Observed the handwashing sink in the kitchen restroom not fully sealed to the wall.
45 - Observed non-NSF/ANSI approved equipment: Mini reach-in refrigerator and coffee grinder. - Observed missing handle on the prep cooler lid. - Observed unused refrigeration unit in the kitchen area.
46 Food facility lacked sanitizer test strips to monitor sanitizer concentration. **QAC sanitizer test strips were provided at the time.
49 Observed the pipe under the sanitizing compartment of the 3-bay sink leaking.
51 Customer/staff restroom: dusty ceiling vent/fan cover observed.
53 Observed opened/unsealed lines behind the prep table in the kitchen area.
53 Observed broken glass and food debris on the floor behind the refrigeration unit in the kitchen area. **Debris and broken glass was swept up at the time.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. *City certification is not on site, but PIC is City Certified. Application for duplicate to be emailed.
8 Sanitary towels are lacking at coffee bar handwashing sink. *PIC provided towels
25 CBD extract was being offered as an addition to prepared beverages. *PIC agreed to discontinue the service of CBD-infused food or beverage.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible in the open-top Avantco refrigerator.
35 Prepackaged food (oats mix) is not labeled properly with distributed by statement. * Place of business of the manufacturer, packer or distributor is lacking.
36 Outer opening in the food facility does not protect against the entry of insects, rodents, and other animals: front garage roll up and door are open/unscreened.
45 Equipment surfaces are not smooth & easily cleanable: some raw wood surfaces under bar counter. Equipment does not meet NSF or ANSI-Sanitation standards: blue refrigerator.
45 One undercounter refrigerator behind coffee bar is not in good repair (TCS/PHF food/beverages stored here were discarded).
49 Approved, indirect drain connections could not be observed (for condensate/drain lines of ice maker and beverage equipment behind coffee bar, as needed). An approved indirect drain (with 1 inch minimum air gap) is not provided at drain line of food prep. sink.
51 A lidded waste receptacle lacks in employee restroom (currently out of use).
