Note: Beverage observed stored directly on the floor in retail, storage and walk-in cooler.

13 Several uncovered food observed in the walk-in cooler and cold prep unit. a dish of cooked rice was observed covered with a re-useable shopping bag.
Note: All food must be properly cover prior to storing them. Food were covered and rice discarded during time of inspection.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
