1 The person observed with a ServSafe Certificate and not with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. *Food safety certificate application was sent via email.
8 The food facility does not have a handwash sink conveniently located in the food serving area. Variance application emailed.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required.
37 -Beverages and Food is not stored at least 6 inches above the floor in the storage room and walk in freezer.
41 Utensils and dishes observed with food contact surface up and not inverted properly with food contact surface down.

1 The person observed with a ServSafe Certificate and not with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. *Food safety certificate application was sent via email. ***Manager stated that there are new staff in the kitchen since last inspection.
13 Chicken in cooler leaking (blood) on the floor not far from vegetables stored in open boxes.
14 Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Person in charge used chlorine as sanitizer as requested from Sanitizer. All dishes were put on the side to be rewashed.
35 Containers in the the walk in container were not properly labeled when removed from its original container.

21 A large container of turkey gravy, dated 1/23, was held in the walk-in cooler passed the allowed 7-days. The container of gravy was discarded during the inspection.

14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. The QUAT sanitizer solution was manually adjusted during the inspection so it measured at 200 ppm.
36 The rear door to the food facility does not protect against the entry of insects, rodents, and other animals. The door was propped open during the inspection and the door sweep was in poor repair.
