26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the ware-washing area. **PIC took corrective action and moved chemicals to a chemical only designated area.
36 Visible physical evidence of rodent activity (mouse feces) observed around the perimeters of the floor in food prep and storage areas.
45 The following were observed: -Raw wood legs which are not smooth and easily cleanable observed being used to support equipment -Milk crates being used for elevation in storage room area which are not smooth and easily cleanable
45 The following were observed: -Closed display case being used for dry storage -Non-NSF/ANSI certified equipment

14 Black residue observed on the interior of the ice machine bin. **PIC was instructed to discard ice and properly wash, rinse, and sanitize interior surfaces of ice bin.
36 Visible physical evidence of rodent activity (mouse feces)/insect activity (fruit flies/house flies) observed in the several areas of the food facility, around the perimeters of the floor, and on exterior surfaces of some shelving units (non-food contact).
43 Single-use articles observed improperly inverted.
