1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
6 Observed food employee using personal phone with gloves, then went back to prepping food without washing hands or changing gloves.
8 Observed the following:
- Paper towels were not available at handwashing sink in women's restroom.
- Dirty dishes inside of the handwash sink in food prep area
11 Observed the following:
- 6 expired baby rice cereals
- 2 expired jars of baby food
- 1 dented can
- 3 expired Gerber apple juices
- 2 expired Similac baby formulas
Corrected on Site: All items were disposed of upon request.
20 Observed deli meats and cheeses held at between 43-45 degrees F rather than 41 degrees F or below as required.
21 Opened commercially processed ready to eat food (Deli meat and cheeses), located in the closed display case, and held more than 48 hours, was not marked with the date it was opened.
45 The following observations were noted:
-Cardboard being used as shelf liner in reach-in refrigeration unit.
-Soda crates being used for elevation
46 Observed food employee using three basin sink to prep vegetables.
-Provide a food prep sink, apply for variance, or purchase prepacked/prewashed vegetables
47 The following observations were noted:
-Debris and residue on internal surfaces of cold prep unit and chest freezers.
-Ice accumulation present in chest freezers.
53 The following observations were noted:
-Debris along floor perimeter under cookline equipment.
-Debris along the floor perimeter of the rear storage area.
61 Beer and Liquor observed, current license is a 3120.

1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. Sanitizer was prepared in the wash compartment.
6 Food employee observed donning single use gloves without a prior hand wash.
8 Observed the following:
- Soap was not available at the handwash sinks in food prep area, or men's restroom.
- Dirty dishes inside of the handwash sink in food prep area
** Soap was provided at handwashing sinks, bowl was still in sink at the end of inspection.
13 Observed uncovered deli meat and bacon inside of closed display case.
20 Observed deli meats and cheeses held at between 43-45 degrees F rather than 41 degrees F or below as required.

1 No food safety person present during arrival of inspection.
Corrective action: Food safety person arrived prior to completion of inspection.
4 An employee beverage container was observed in the cold prep unit in the food preparation area.
8 The handwash sink in the bar area was blocked by a table and not accessible at all times for employee use.
8 Paper towel dispenser empty at the handwash sink in the food prep and bar areas.
11 2 Gerber oatmeal's observed expired with dates July 2024.
Corrective action: PIC discarded during the time of inspection.
13 Knives and utensils observed being stored in the crevice between the 3-basin sink and wall surface.
Corrective action: PIC removed knives and utensils for washing, rinsing and sanitizing.
14 Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.

26 Observed rodenticides (Mouse Bait) not labeled by the manufacturer for food facility usage, in multiple areas throughout the facility.
27 No food prep sink with an indirect waste drain and catch basin observed in the food prep area.

34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible.
