8 -Handwashing sink in the rear food prep area has a fast leak at the drainage pipe. Corrective action: Maintenance request was made and sink was repaired during report writing. -Trash can and tray rack were observed in front of the handwashing sink in the warewashing area. Corrective action: Items were relocated.
14 Slime observed on the interior of the ice machine bin on the ice guard. Corrective action: Machine was emptied and cleaned during the inspection.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food. Corrective action: Per food employee, pizzas came out at 11:00 a.m. Log was completed during the inspection.
45 -Open display case in the point-of-sale area is holding food above 41 degrees F and should not be used to hold potentially hazardous food. -The following inoperable equipment was observed in the food prep area: warmer/hot holding unit, under-the-counter refrigerated drawer unit.
47 Cleaning required: -Some grease observed on top of the refrigerated drawer unit holding the flat grill.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. No food safety manager was present at the start of the inspection. Corrective action: Food safety certified person T.Q. Hoang arrived at approximately 11:30 a.m.
11 Severely dented, swollen, distressed canned item observed in dry strorage area. Corrective action: Can was moved to area for returns. Note: Please provide signage in designated area for food that is being returned to the distributor.
11 Water observed dripping down the interior wall of the ice machine bin onto ice. Black residue observed on exterior wall. Corrective action: Ice was discarded during the inspection.

14 -The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Indicator showed 99 to 108 degrees F. Instructed the person in charge to discontinue using the dishwasher and to use the 3-bay sink.
