1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.

36 Mouse feces observed on floor, corners, and perimeter of the pool storage, the food preparation/dining room, Video room (corner), and inside dining room cabinets. Rodent harborage and clutter observed in the pool/supply storage room.

45 Two microwave ovens observed in food prep and front end area are not NSF/ANSI approved.

45 One reach in refrigerator in the food prep area observed inoperable.

47 Reach in refrigerator in the Art room, interior observed with residues and unclean to sight.

51 The women's toilet room lacks lidded receptacle for sanitary napkins. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

53 Acoustic ceiling tiles throughout the facility observed damaged, missing, and water damaged. Ceiling is not smooth or easily cleanable.
