3270.65 Several (7) outlets were lacking protective covers in the infant, 2-3 year old class and young toddlers class room. Some protective covers were provided.
3270.66(a) Cleaning products were stored on open shelves in the toilet room also used by the children and not locked in a cabinet to prevent children touching these items. For example, bleach, Fabuloso, Pine cleaner, and other products. The inspector requested that these bottles be moved to a locked cabinet.
3270.76 Porous and water stained ceiling tiles in the toilet room.
3270.67(c) One dead roach observed in the basin sink of babies' Room.
3270.82(g) The infant room hand sink cold water faucet is damaged and loose. Low water pressure and flow for the cold water observed. The hot water generation is slow at this hand sink.
3270.102(a) A small flat screen television is not anchored in a toddler classroom. A child pulled the television onto the floor during the inspection. The inspector requested that the television be removed. The teacher removed the television. Corrected On Site
3270.104(a) -Dust and dirt were observed on safety gates.
3270.106(c) Observed a child laying on a stack of mats rather than one designated mat. The teacher took the mats apart during the inspection. Corrected On Site

1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. She could not recall the names of the foodborne illnesses that should be reported. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. The person in charge has a class certificate but no Philadelphia Food Manager Food Safety Certificate. The application will be emailed with this report.
8 -Soap and paper towels were not available at the handwash sink in the infant room. -There was no trash can at the food prep area hand sink. The required items were provided.
13 Two bags of whole apples were observed in the refrigerator. There is no dedicated indirectly drained food prep sink equipped with a catch basin. Request prepackaged pre-washed fruits and produce. The two bags of apples were discarded.
20 The food prep area refrigerator is not maintaining foods at 41 degrees F or lower throughout the unit. Observed 2 boxes of fish sticks probed at 50 degrees F rather than 41 degrees F or below as required in the food prep area refrigerator. The person in charge was instructed to dispose of these boxes due to fish stick having been stored in this unit for more than four hours.
34 A thermometer was not observed inside the Infant Room mini refrigerator.
45 Equipment is not in good repair or approved for commercial use: -The cold faucet at the hand sink in The Infant Room is damaged and loose and not supplying water adequately. -The food prep area refrigerator is accumulating ice in the rear. The ambiant air temperature on the top shelf of this unit was scanned at 50 degrees F. The freezer in this unit is not working. No food was observed in the freezer. -The microwave and mini refrigerator in the Infant Room are not commercial or NSF approved. -The portable drain rack in the 3 basin sink is not commercial.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
47 Observations: - Accumulated ice on the back inner surface of the food prep area refrigerator. -Residue observed in the drawers of the refrigerator in the food prep area. -General debris on inner cabinet surfaces in the food prep area. -Dead roach in the basin of the hand sink in the toddler classroom.
48 - Slow hot water generation at the Toddler Room hand sink.
49 Plumbing issues: - There is no dedicated indirectly drained food prep sink equipped with a catch basin for the washing of fruit and produce. - The faucet sprays and leaks at the hand sink in the kitchen area. - The 3 basin sink is not sealed to the wall.
49 No mop sink (utility sink) observed in the daycare center.
53 -Stained, damaged, and porous ceiling tiles observed in the facility. -Loose coving tiles observed in classrooms.
53 -Residues and debris are observed on floor perimeters and surfaces in classrooms. - Dead roach observed in the hand sink in The Infant Room.
54 Employee purse observed on the floor rather than in a designated area away from the children in the Infant Room. The teacher hung her purse on the provided hooks in the classroom.

1 -The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. -No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. A person with a Food Manager Food Safety Certificate arrived within the alotted time.
8 No paper towels at the handwash sinks in the food prep area and toilet room areas. Paper towels were provided.
11 Two dented canned food items observed in office storage area. Carrots and Green Beans. Inspector requested that the cans be returned to the distributor.
13 Whole apples observed in the refrigerator. There is no dedicated indirectly drained food prep sink equipped with a catch basin. Provide prepackaged fruit and produce. The bag of apples were discarded.
14 Not washing, rinsing, and sanitizing dishes, equipment and utensils. -Observed no chemical sanitizer being used on baby bottles after being washed. -The person in charge set up the sink to rinse, wash, and sanitize rather than to wash, rinse, and sanitize. The inspector reviewed the proper way to set up the sink. The person in charge correctly set up the 3 basin sink and located test strips to test the solution.
20 The mini refrigerator in the Infant Room is not maintaining milk at 45 degrees F or lower. The milk in the refrigerator was held at 59 degrees F rather than 41 degrees F or below as required. The person in charge was instructed to dispose of milk due to milk not being maintained below 41 degree F. Do not use this refrigerator until it can maintain a temperture of 41 degrees F or lower.
26 Observed insecticides (Raid spray) not labeled by the manufacturer for food facility usage, in the infant room area. All pest control should be done by a licensed exterminator. Raid spray should be removed from the facility.
26 Employee medicines (Advil) stored in the infant room on a shelf along other food items, and were not stored to prevent the contamination of food, equipment or utensils. The employee moved the bottle to her purse.
26 The bleach that was readily available was not labeled for santizing food contact surfaces. The person in charge obtained the proper bleach while the report was being typed.
34 A thermometer was not observed inside the Infant Room mini refrigerator.
36 Visible physical evidence of insect activity (flies and roaches) observed in the Young Toddlers Room in the hand sink cabinet.
39 Moist wiping cloths observed lying in the 3 basin sink and not stored in sanitizing solution.
42 Plates in the Kitchen area are stored upright, instead of inverted to avoid contamination.
43 Plastic single-use bowls being used to warm other foods. Multi-use bowls should be used or use single-use bowls once and discard.
45 Some cabinet in classrooms and office room observed broken
45 Equipment is not in good repair or approved for commercial use: -The pipe connecting the waste container to the hand sink is not connected to the sink in The Infant Room. -The freezer is not working. -The microwaves (2) and mini refrigerator in the Infant Room are non-NSF approved.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
47 Observations: -Dead roaches observed on surfaces in the food prep area. For example, under and around the microwave, on the hand sink and the cabinet below the hand sink in the food prep area. -Dead roaches, live roaches and flies observed on the waste container below the hand sink in the Young Toddlers Room.
49 Plumbing issues: -There is no dedicated indirectly drained food prep sink with a catch basin for the washing of fruit and produce. -The faucet leaks at the hand sink in the kitchen area,. -The hand sinks in the diaper stations are not properly maintained - The 3 basin sink not sealed to the wall
49 No mop sink(utility sink)observed. A toilet in the toilet room is being used to dispose of mop water.
53 -Stained, damaged, and porous ceiling tiles observed in the facility. -Loose coving tiles observed in classrooms. -Opening in wall surface in Young Toddler Room.
53 -The hand sink waste container in the Young Toddler Room is not being emptied often enough to prevent the attraction of insects. -Residues and debris are observed on floor perimeters and surfaces in classrooms. - Dead bugs were observed on the kitchen sink and under the sink in The Infant Room. -Stagnant water with bad odor observed under hand washing sink in The Infant Room.
53 Brooms stored on the floor rather than hanging on the utility rack. They were hung on the rack.
54 Employee personal medication observed stored baby's items rather than in a designated area away from the children.

3270.65 Several outlets were lacking protective covers. For example, 2-3 year old class, hallway, older toddlers class, and school age room that is connected to older toddlers room. Some protective covers were provided.
3270.66(a) The closet in the infant room was not locked and in poor repair and contained several bottles of cleaning solutions (for example, bleach, Fabuloso, Pine cleaner, and others) stored on the floor. The bottles were moved to the office area. Corrected On Site
3270.66(c) Raid insect spray observed on a shelf in the Infant Room. All pest control should be done by a licensed exterminator.
3270.76 Observations: Dead roaches, live roaches, and flies observed in the Young Toddler Room under the hand sink. Loose coving throughout the facility. Dead roache on surfaces in the food prep area. Openings in surfaces in the facility. Water stained and damaged porous ceiling tiles. For example, toilet room. Built up residue observed on the floor perimeters near the wall in classrooms.
3270.67(c) -Dead roaches observed on surfaces in the food prep area. -Dead and live roaches and flies observed in the cabinet below the hand sink in the Young Toddlers Room.
3270.75(c) Scissors were not available in the Infant Room First Aid Kit. They were provided. Corrected On Site
3270.82(g) The hand sink in the Young Toddler Room is in poor repair. The pipe is not connected to the sink. The waste tank contained dirty water with an odor. Roaches and flies observed in the cabinet around the dirty waste tank. The person in charge emptied the tank and stated that he would clean and sanitize the waste container. Diaper changing and the feeding of children is done in this classroom. The teacher and children were moved to a classroom with a handsink.
3270.102(a) Surfaces throughout were not clean to sight and touch. Static dust and dirt were observed on safety gates, cubby surfaces, and plastic drawers. Damaged cushions observed in the school age room. The cushions were discarded.
3270.104(a) Stacked plastic drawers were not secured to the wall to prevent tipping. The plastic drawers were not assembled properly. The person in charge removed the top plastic drawers off of the lower plastic drawers. Corrected On Site
3270.135(b) The concentration of bleach was 0 ppm in the Young Toddler classroom.
