41 Food dispensing utensils in bulk ingredient storage containers observed in direct contact with food and not having handle stored above the top of the food.
45 Non-operational reach-in refrigerator (Coke) in the food prep area.
47 The following were observed:
-Food debris on lower interior surfaces of shelving units in food prep area
-Food debris on lower interior surfaces of under-the-counter refrigeration unit in food prep area
-Food debris on lower interior surfaces of reach-in refrigeration units in rear of food prep area
51 The following were observed:
-Residue around faucet handles of handwashing sink in unisex employee restroom area
-Residue around interior surfaces of sink in the unisex employee restroom area
-Static dust accumulation on air vent in unisex employee restroom area
51 A lidded receptacle for sanitary napkins was not observed in the unisex employee restroom area.
**A restroom used by females shall be provided with a lidded receptacle for sanitary napkins.
53 Perimeters of floor in food prep and dry storage area have an accumulation of food debris/residue.
53 Brooms observed stored on floor and not elevated on a utility rack designated for maintenance tools.
53 Mop observed not elevated for proper air dry.

1 Food Safety Certified person was not present at the start of inspection.
**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Food Safety Certified person arrived by end of inspection.
26 Chemicals were stored above food on dry storage shelving unit.
**PIC took corrective action and stored chemicals in a chemical only designated area during time of inspection.
