1 Food Safety Certified person was not observed with a Philadelphia Certificate.
**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**An application for the Philadelphia Certificate was issued to Food Safety Certified person during time of inspection.
8 Old food debris/residue observed in the handwashing sink in rear of food prep.
**PIC took corrective action and discarded food debris and properly washed, rinsed, and sanitized interior surfaces of handwashing sink during time of inspection.
8 Paper towel dispenser empty at the handwashing sink in the men's employee restroom area.
**PIC took corrective action and provided paper towel during time of inspection.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food for chicken located in walk-in cooler.
**PIC took corrective action and provided time stamps during time of inspection.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in reach-in freezer in food prep area.
36 Visible physical evidence of rodent activity (old mouse feces) observed around perimeters of floor front corner of food prep area near safe and drive thru window and behind toilet in men's employee restroom area.
**PIC took corrective action and discarded mouse feces and properly washed, rinsed, and sanitized perimeters of floors during time of inspection.
45 The following were observed:
-Missing front cover upper surfaces of ice machine
-Broken hand dryer in men's employee restroom area
47 The following were observed:
-Food debris around perimeters of floors in walk-in cooler/freezer
-Food debris on lower interior surfaces of reach-in freezer
-Grease accumulation on interior and exterior surfaces of cooking equipment
-Food debris on lower interior surfaces of hot holding units
51 Old mouse feces observed around perimeters of floor in men's employee restroom area.
**PIC took corrective action and discarded mouse feces and properly washed, rinsed, and sanitized perimeters of floors during time of inspection.
53 Food debris/residue, grease accumulation, stagnated water, and old mouse feces observed around perimeters of floor in food prep area and men's employee restroom area.
****PIC took corrective action and discarded mouse feces and properly washed, rinsed, and sanitized perimeters of floors during time of inspection.
53 Brooms and dustpans observed stored on the floor and not elevated on a utility rack designated for maintenance tools.
53 Mop observed stored in a bucket of stagnated water and not hanging in a manner that facilitates air drying.
53 The following were observed:
-Stained ceiling tiles in food prep area
-Cracked floor tiles in food prep area
54 The following were observed:
-Six lights in the food prep area are not shielded from breakage
-One light out in walk-in freezer
-One light out under exhaust hood
