14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 150 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions.
QAC pills added to solution to derive 200 ppm during the inspection.
21 Seafood salad opened commercially processed ready to eat food, located in the reach-in refrigerator, and held more than 48 hours, was not marked
with the date it was opened.
Date was put on seafood salads during the inspection.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
39 Moist wiping cloths observed lying on prep table and not stored in sanitizing solution.
43 Single-use disposable take-aways observed not inverted or covered before use to prevent contamination.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

-Loose rubber door gaskets observed on the standing refrigeration unit.
-Unplugged closed display case and ice machine observed turned off in the front prep area and not in use.
-Hot water faucet observed not properly closed and leaking during the inspection.
