**PIC took corrective action and removed the milk from reach-in refrigerator.
13 - A pan with deli meat observed stored above ready to eat foods (salad) in the cold-prep Unit cooking area.
*Sanitarian instructed the person in charge to remove the pan with deli meat and discarded the salad.
14 The can opener had an accumulation of food residue on the food contact surfaces.
35 Working bulk food ingredient storage containers (sugar and pasta) are not labeled with the common name of the food in the food prep area.
36 The following observations were noted:
- Visible physical evidence of rodent (mouse droppings) observed behind the counter in the front prep area, behind the water-heater and along the floor perimeters in the dry storage areas.
-Dead cockroaches on 1 open mouse trap.
45 Plastic crates were used for beverage elevation in the retail area.
45 - Raw wood being used for elevation in the walk-in cooler were not smooth and easily cleanable.
46 The 3-compartment sink was observed to have 1 missing stopper.
53 The following observations were noted:
-Gaps/holes observed around utility lines in the facility.
-Damaged/uneven flooring tiles observed in the storage room.

PIC provided the soap and paper towel during inspection.
13 - Uncovered food (chicken tenders) observed in the walk-in freezer.
*Sanitarian instructed the person in charge to cover the food in the walk-in freezer.
14 The can opener had an accumulation of food residue on the food contact surfaces.
PIC took corrective action and washed, rinse, and sanitized the can opener during inspection.
19 - Cooked food (breakfast sausages) observed in the hot holding unit at 114 degrees F.
*Sanitarian instructed the person in charge to increase the temperature for the hot holding unit.
20 The closed display case in the prep/retail area observed at 50 degrees F.
Food (pasta salad, chicken salad, cooked chicken, lunch meat, and cheeses) where removed to walk-in cooler during inspection.
21 - Several food items (pasta sauce, burgers) observed without the date in the walk-in cooler.
PIC provided dates for the food during inspection.
36 The following observations were noted:
- Visible physical evidence of rodent (mouse droppings) observed on the first and second shelves front prep area and along the floor perimeters in the storage areas.
- House flies observed in the retail area.
36 - An insect control device (fly glue strip) observed in storage area.
PIC removed the fly glue strip during inspection.
37 Buckets with food observed on the floor in the walk-in cooler.
PIC elevated the buckets with food in the walk-in cooler.
45 Cardboard observed lining over shelves /storage rack, and wooden palettes being used for elevation in the walk-in cooler were not smooth and easily cleanable.
45 Non-NSF approved equipment observed: porcelain handwash sink in the kitchen.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
51 Toilet seat observed in need of cleaning in the customer's restroom.
53 The following observations were noted:
