45 - Inoperable ice machine observed in kitchen area. -Defective chest freezer in basement area.
45 Domestic microwave observed in kitchen/prep area. * No personal usage sign observed.
47 -Food debris observed on floor surface of walk-in cooler. - Dust accumulation observed on fan covers in walk-in cooler. - Glue traps observed on floor surface of walk-in cooler with dead bugs.
49 - Continuous drip observed at hand wash sink faucet in bar area. - Hot water side of faucet observed loose in women's restroom.
53 The following observations were made : - Food debris on floor surface in prep area. -Food debris and dust under equipment in bar area. - Debris, dust and broken glass in bar area under equipment. -Dead mouse observed on glue trap in basement area. * Glue trap was discarded.
53 - 2nd floor observed not in use. -Miscellaneous organized clutter observed in rear of basement.
53 - Damaged floor tiles observed in front of hot hold unit in kitchen area. -Syrup spill on floor surface under soda gun storage rack in basement.
53 Broom and dustpan observed not elevated off of the ground when not in use.
53 Mop observed stored in mop bucket of water and not hung to allow for proper air drying when not in use.
54 Hood filters have an accumulation of static dust and grease.

8 No soap observed at hand wash sink in bar area. * Soap was provided.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints. * All employees put on hairnets during inspection.

1 The Person in Charge did not demonstrate adequate knowledge (Food borne illnesses) of the Philadelphia Food Code. * The inspector reviewed food borne illnesses with the person in charge.
8 The 2nd hand wash sink in the bar area was blocked by case of soda and not accessible at all times for employee use. * The person in charge cleared hand wash sink.
37 Case of water observed not stored off of the ground in the basement. * Employee elevated case of water onto storage rack.
45 Food prep sink observed loose and not properly sealed to wall surface.

14 The can opener had an accumulation of food residue on the food contact surfaces. * The can opener was removed and properly cleaned and sanitized.
19 Food (Rice) was held at 130 degrees F rather than 135 degrees F or above as required on steam table. * The person in charge reheated rice. -Rice observed at 142 degrees F.
36 Visible physical evidence of rodent activity (Mouse droppings) observed in the kitchen area on bottom surface of prep table. * Droppings were cleaned up and table was sanitized. - Droppings observed in bar area on flooring surface of alcohol storage. - Some mouse drippings observed behind chest freezer in basement.
