1 Facility's food safety certified person lack of the City of Philadelphia food safety certificate; Observed ServSafe certificate only. * Advised facility's food safety certified person to apply for the City of Philadelphia food safety certification. City of Philadelphia application e-mailed on 8/21/2024. Corrected On-Site.
8 Missing hand wash sink in the restroom area. Observed a polypropylene freestanding laundry/utility sink used as hand wash sink in the restroom area.
8 Paper towel dispenser empty at the handwash sink in the service area. Soap was not available at the handwash sink in the cooking area.
13 Uncovered food (chicken, beef, pastry, etc.) observed in the reach-in refrigerator, under-counter refrigerator, and reach-in freezer.
13 Dough observed stored on the counter on dirty knives, dough cutters, and rusty box cutter.
14 -Meat grinder has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
14 Dough mixer has an accumulation of food residue and is not being washed at least every 24 hours.
14 Food preparation sink was observed unclean to sight and touch.
14 -The warewash (3-compartiment) sink not properly setup for sanitation of utensils and equipment; the person in charge set up the sanitizer bay on the second compartment. * Corrective action: Sanitarian instructed the PIC on how to properly setup the 3 -comp sink for sanitation of utensils and food equipment. A return demonstration of Wash-Rinse-Sanitize setup was observed prior to completion of inspection.
14 A hair rubber band observed on the cutting board / kitchen area. PIC remove the hair rubber band but did not sanitized the cutting board.
20 - Food (corn and sandwiches turkey and cheese) were held between 62-71 degrees F rather than 41 degrees F or below as required; corn observed in the plastic bin on the counter /prep area and sandwiches where stored inside the display case /service area.
21 No dates observed on food in the facility. *Sanitarian instructed the person in charge to provide dates for all the food in the facility.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods (sandwiches) without written procedures or documentation to verify disposition of food.
