1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
6 Food employees shall clean their hands and exposed portions of their arms. - (1) Immediately before engaging in food preparation activities such as working with exposed food, working with clean equipment and utensils, and working with unwrapped single-service and single-use articles. - Microwave and toaster observed in infant/1st floor classroom. No hand wash sink observed present for employees to properly wash hands. **All items should be removed.
35 Container of Sugar observed on 2nd floor storage table with no label. **Working containers holding food or food ingredients (such as cooking oils, flour, herbs, potato flakes, salt, spices and sugar) that are removed from their original packages for use in the food facility shall be identified with the common name of the food they contain
45 - Non-NSF approved equipment observed (refrigeration units, microwave, Toaster. No employees use only sign observed. - Unused warming unit in 2nd floor food storage area.
47 Food debris observed on bottom surface of reach-in freezer.
51 Damaged/missing cove base observed in women's restroom.
53 Broom and dustpan observed not elevated off of the ground when not in use.
53 Debris and dust along floor perimeter in 2nd floor food storage area.

3270.166(4) Bottles in reach-in refrigerator observed not labeled with children's name. The person in charge wrote names on bottles.
