1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
There was no Food Safety Certified person present at the beginning of the inspection.
He arrived within the allotted time.
8 -The handwash sink in the food prep area was blocked by boxes and plastic in the basin and not accesible at all times for employee use.
-Soap, paper towels, and a waste container were not available at the handwash sink in the food prep area.
The required items were provided at the sink.
8 The hand sink in the food prep area was lacking hot water at the beginning of the inspection.
The person in charge turned on the hot water at the valve below. The faucet is leaking.
13 Raw bacon stored over an open bowl of egg mixture in the cold prep refrigerator.
The bowl of egg mix was removed from the refrigerator.
14 Not washing, rinsing, and sanitizing dishes, equipment, and utensils. There was no sanitizer readily available at the 3 basink sink.
Sanitizer tablets were obtained during the inspection. The person in charge set up the 3 basin sink.
21 Deli meats and cheeses, opened commercially processed ready to eat food, located in the closed display case, and held more than 48 hours, was not marked with the date it was opened.
An employee began writing dates on a piece of paper to be displayed at the closed display case.
26 Chemicals (alcohol, hand sanitizer, WD40 oil, personal care items, cleaning solutions) were stored above or on the same shelf with food, equipment, and/or single service articles in the cashier, retail, and food prep areas.
Employees moved the chemicals away from the food items on the shelves.
34 Temperature measuring devices (probe thermometer) for ensuring proper temperatures of food are not available or readily accessible.
35 Prepackaged cookies, crackers, and soft pretzels were not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
The soft pretzels were removed from the cashier area.
37 Boxes of potatoes and onions were not stored at least 6 inches above the floor in the retail area.
45 -Unfinished wood shelving observed in the rear food prep area.
-Card board liners in the retail and food prep refrigerator is not smooth and easily cleanable.
45 -Damaged and taped lid on the chest freezer in the food prep area.
-Garbage disposal installed in a shallow and wall mounted handwashing sink. -Crates used for item storage are not approved shelving.
-Cover is missing from refrigeration unit compressor. Condensate drips into a container(at bottom of display case).
-Non-commercial drain rack over the 3 basin sink.
45 Splash guard not provided between the sanitizer basin and the hand/grinder sink.
46 Food facility lacks QAC sanitizer test strips to monitor sanitizer concentration.
46 The non-commercial drain racks above the 3 basin sink are used for other item storage rather than left uncluttered for the proper drying of cleaned dishes, equipment, and utensils.
47 Observation:
-Dust and debris observed on shelving, bulk storage containers, and cardboard liners in the food prep area.
-Food debris and residue inside cold prep refrigerator in the prep area.
-Debris on the tracks of the closed display case.
49 -Observed an indadequate backflow prevention device (meant for an ice machine) at the main water inlet. A double valve RPZ backflow preventer is required.
-The food facility lacks a dedicated indirectly plumbed food prep sink with a catch basin for the washing of produce.
-The garbage grinder is indirectly plumbed rather than directly plumbed.
49 -A utility sink was not observed.
51 Static dust on surfaces in the toilet room.
52 -Observed used boxes of oil and other boxes stored on the ground and not in a lidded container.
-A lidded used oil storage container was not observed in the trash storage area.
The person in charge stated that the oil is taken to the 2700 Snyder Ave location to be stored in that container.
53 -Missing wall covering behind the sinks in the prep area.
-Damaged door and damaged surfaces in the toilet room.
-Domestic porous ceiling tiles. Some are water stained.
-Porous ceiling tiles in the restroom are not smooth and easily cleanable.
53 -Broom, dust pan, and mop are stored on the floor rather than stored elevated on a rack.
53 An idle mop observed stored in a bucket instead of hung for air drying.
53 -Debris and dust observed on the floors perimeter beneath shelves in the retail area.
-Clutter and unused equipment (rusted chest freezer and other items) observed in the basement storage area.
54 No overhead light provided in the basement storage area to enable proper cleaning and inspection.

-Residues and splashes observed on the ceiling tile and light shield in the food prep area.

53 -Ceiling tiles in prep area are (acoustic surfaces) difficult to clean, stained, and missing. Holes observed in the ceiling of the rear retail and prep areas.

8 The hand washing sink in the food prep area was observed blocked with cleaning products and utensils. A handwashing facility shall be maintained so that it is accessible at all times for employee use.
The person in charge removed all of the items from the hand wash sink.
13 -Deli meats inside of the closed display case observed uncovered.
-Bacon, cooked potatoes and other meats in the under counter refrigerator observed uncovered.
Corrective action was not taken at the time of inspection.
14 The 3-bay sinks were observed clogged with water and grease. The facility cannot perform wash, rinse, and sanitizing practices for utensils and equipment as required. The person in charge unclogged, and was able to wash, rinse, and sanitize in the 3-bay sinks by the end of the inspection.
