14 - The can opener had an accumulation of food residue on the food contact surfaces. * The can opener was removed and relocated to 3 compartment sink to be washed, rinsed and sanitized.
20 Potentially hazardous food items (Tomatoes, Cucumbers, Carrots, Guacamole, Cheese, Corn, Peppers, mandarin oranges, Sauces, Onions, Lettuce, Beans & Strawberries was held at 46-56 degrees F rather than 41 degrees F or below as required. ** Food items were discarded.
34 Thermometer observed missing inside of pizza prep refrigeration unit.
36 - Visible physical evidence of insect activity (small flies) observed in the basement area near syrup pump area. - Small flies observed in ware wash area. **The person in charge provided email of pest control services received today.
45 - Walk-in cooler observed inoperable and not holding a temperature of 41 degrees F or below. *No PHF items observed. Remaining food items were relocated to beer walk-in box. -Flooring surface of walk-in freezer observed damaged upon entry. - Two cold prep/Under counter refrigeration units observed defective and not holding temperatures of 41 degrees F or below in prep area.
46 Food facility lacks correct sanitizer test strips (Quat) to monitor sanitizer concentration.
